Main duties of the job
The role includes home visiting; providing medicines management interventions to patients within their own home, involving lone working without direct supervision. You will liaise with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network with a focus on promoting independence, prevention and early intervention. Delivering medicines management training to relevant staff groups will also be included, and participation in audit processes .
The post holder must be able to travel around the locality.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply

- Effective interpersonal, communication (both written and oral), presentation and influencing skills

- Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
Essential Qualifications & Knowledge

- Registered with relevant professional body such as NMC or GPhC
- Specialist knowledge acquired through health related qualification such as BTEC science (pharmaceutical) Registered Pharmacy Technician, equivalent level qualification plus specialist training and experience to degree or equivalent level
- Knowledge and evidence of Professional requirement for ongoing Continuing Professional Development to maintain knowledge and expertise

- Further post graduate management or clinical qualification
