Fixed Term - 1 year
This role supports individuals with potential or diagnosed cancer.
The post holder will be an identified and consistent point of contact for patients and speciality teams to manage patient cancer pathways.
To provide essential coordination between hospital departments/services to ensure patients are kept informed to reduce anxiety and ensure efficiency within the cancer pathway. To ensure waiting time targets are achieved and to escalate where required.
In conjunction with the Clinical Team, coordinate access to the right information and education resources to support patients throughout the complex cancer pathways.
Develop a partnership approach to working to empower the patient and carers.
