The Gastroenterology service has links to the upper gastrointestinal / HPB surgery at HH and colorectal at CXH. It is linked with Histopathology and Radiology, as well as Dietetics, Bariatrics and hepatology teams. The endoscopy units at ICHT provide diagnostic and therapeutic interventional services. All 3 sites are JAG accredited. Services include upper and lower GI endoscopy, bowel cancer screening, capsule endoscopy, ERCP, EUS and stenting As well as radiofrequency ablation, enteroscopy, cholangioscopy and EUS.

The Gastroenterology Consultants at St Mary’s Hospital provide ward cover on a rotational basis. There is a consultant-led nutrition round on each site on a weekly basis. The post has an on-call Acute Medical Take, approximately 1:30. At the St Mary’s site there is a weekly combined medical/surgical/radiology meeting, and a weekly departmental meeting. There are also a cross site MDTs for inflammatory bowel disease, polyps, colorectal / upper GI cancers. Consultants are provided with office space and secretarial support, as well as access to ICT support.

Main duties of the job

The post holder will work closely with other members of the team to ensure high quality acute and general medical and gastroenterology services within the strategic framework agreed by the team.  They will work closely with the appropriate clinical management teams.

Ward responsibilities, and acute take responsibilities, will be as described in section 2.2 above.

The post holder will undertake 5 endoscopy lists / outpatient clinics per week.

There will be a commitment to the out-of-hours endoscopy service.

Provide High Quality Care to Patients in Acute & Specialist Medicine

  • Ø  The post holder must be medically qualified and maintain GMC specialist registration. Ø  The post holder must be JAG accredited in OGD and colonoscopy. Ø  The post holder must be competent in managing GI bleeds. Ø  The post holder must maintain the competencies required to carry out the duties of the post. Ø  The post holder must ensure prompt attendance at agreed direct clinical care Programmed Activities. Ø  The post holder must ensure patients are involved in decisions about their care and to respond to their views. Ø  The Trust supports the requirements of CPD as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities. The job plan includes 1 PA for Continual Professional Development.

Detailed job description and main responsibilities

In-patient Gastroenterology

The Gastroenterology team at SMH currently provides a consultant ward cover on a 1 in 6 basis. Almroth Wright ward is 15 side room beds. The ‘Consultant of the week’ also is responsible for providing advice for inpatient referrals, performing vetting and managing inpatient GI bleeds while covering the wards weekdays 9am – 5pm.

Each post also has a 1:30 commitment to the Acute Medical Take.

Gastroenterology out-patient services

General Gastroenterology out-patient clinics

  • Inflammatory bowel disease outpatient clinics at St Mary’s, Hammersmith and Charing Cross
  • Specialist HPB clinics at Hammersmith
  • Day case endoscopy including gastroscopy, colonoscopy, ERCP & EUS, balloon enteroscopy
  • Capsule endoscopy (CXH and SMH)
  • Upper GI physiology (SMH)
  • Bowel Cancer Screening

Acute GI Bleeding on call

There is an on-call consultant who covers all three sites for Upper GI Bleeding out of hours. The post holders will not have a commitment to this rota, but will be expected to be confident managing emergency inpatient GI bleeds including variceal therapy.
