56 Dean Street is an award winning HIV and sexual health service based in Soho with a particular emphasis on HIV prevention and the sexual health care needs of LGBTQI+ people. It is Europe’s busiest sexual health clinic with an attendance of approximately 110,000 patients per year. It cares for the third largest HIV cohort in the country with approximately 4,600 patients. 56 Dean Street has been commissioned to provide TransPlus, the first integrated gender, HIV and sexual health service by NHS England. Our vision is for an innovative and patient-centred service, based on the principle that patients must have equal access to services whatever their gender identity with reasonable adjustments to the delivery of care to match the individual’s needs and circumstances.

The post is for a 10 PA Consultant in Gender Health, HIV Medicine and Sexual Health within the TransPlus gender dysphoria service, for 12 months fixed term. The job is based at 56 Dean Street, Soho.

Main duties of the job

The post holder will provide assessment and diagnosis of gender dysphoria, in the role of a ‘lead clinician’ as per Appendix H of the NHSE Service specification: Gender Identity Services for Adults (Non-Surgical Interventions). They will formulate treatment plans for those who meet the diagnostic criteria, as well as providing endorsement for gender affirming surgery where appropriate. They will work closely with a multi-disciplinary team of nurses, pharmacists and resident doctors to provide holistic and safe care for the duration of the patient’s care under TransPlus. They will regularly attend multi-disciplinary meetings to discuss complex cases and ongoing service delivery. It is anticipated that the post holder will partake in research and audit within TransPlus.

Additionally, the post-holder will, in conjunction with the existing consultants, have responsibilities for providing advice, screening, diagnosis and treatment of patients with STIs, including HIV infection. They will be committed to developing integrated HIV & sexual health services, optimize cost efficiency of the service, implement new technologies and develop new initiatives.

Detailed job description and main responsibilities

  • Provide assessment and diagnosis of gender dysphoria
  • Formulate and monitor treatment plans for patients diagnosed with gender dysphoria at TransPlus
  • Assess readiness of patients for gender affirming surgeries and refer where appropriate
  • Provide advice, screening, diagnosis and treatment of patients with STIs, including HIV infection.
  • Participate in the local HIV multidisciplinary meeting at 56 Dean Street.
  • Contribute to implementation of clinical pathways
  • Provide consultant advice and support to multidisciplinary team working in Gender Health, HIV and GUM clinics. Review complex cases requiring senior input.
  • Manage their own caseload of HIV patients
  • Provide supervision and support for nursing colleagues and junior medical colleagues providing management of patients with gender dysphoria, stable patient cohorts, management of seroconversion, starting/switching treatment and management of complex or unwell patients.
  • Work with colleagues to modernize the HIV care pathway to ensure effective use of resources.
