Dartford and Gravesham NHS Trust is delighted to be able to offer an opportunity for a highly motivated, ambitious, dynamic individual to join our committed, expanding and providing high standard care to the local community as a Consultant Physician in Acute Medicine.
Main duties of the job
Dartford & Gravesham NHS Trust has an Acute Medical Unit (AMU) and a modern expanding Same Day Emergency Care (SDEC).
Acute Medical Unit has 7 consultants. Most of the consultants work less than full time in Acute Medicine and have other speciality interest including renal medicine, endocrine and intensive care.
Darent Valley hospital has a 34-bedded AMU. The AMU is supported by pharmacy and IDT teams’ presence on the ward to help support safe discharges. You will be scheduled to be consultant of the day or support consultant in the morning.
SDEC works within the newly opened SDEC village, which includes Hospital at Home Team (HAHT) and Frailty SDEC services. We see around 50-60 patients through SDEC in a day. Ward In-charge and Nurse co-ordinator, who triage and pull patients suitable for SDEC, support the service.
The appointed candidate will also be part of the GIM oncall rota. In addition, if the appointed candidate has additional interest this could be discussed and reflected in the job plan. Going forward it is envisaged that we will have a rota, which would have an AMU consultant of the week. We also are working to an Acute Assessment unit as per the GIRFT criterion and there is potential opportunity for the new Consultant to help develop the unit.
Detailed job description and main responsibilities
The post holder will join a team of 6 acute consultants. They will take part in
- a) AMU Ward round: Allocated as AMU consultant of the day once/twice a week. To do a post take ward round in the morning with the AMU resident doctors from 8 am. Lead the board rounds around midday with the resident doctors, Nursing team and allied health teams. In the afternoon the consultant will support the ward and resident doctors manage Acutely unwell patients and facilitating safe discharges. b) SDEC: Minimum once a week scheduled to cover SDEC. Shift begins at 0900 and ends at 1800. There will be two board rounds in the day discussing all patients with resident Doctors and nurse in charge. To review all cases of ACP and other resident doctors as appropriate and post take any admissions from SDEC. Currently SDEC consultants support GIM take by doing an afternoon post take round but this would be under review. After 7 pm the A&E consultant will be supporting SDEC upto 10 pm. c) GIM on call: the successful candidate will take part in the GIM on call rota as well.
