Job overview

We have an excellent opportunity within our Enhanced Support and Liaison Service for a Liaison Practitioner (Inpatient pathway). You will be based within the LDA Enhanced Support and Liaison Service and have a dedicated focus on the inpatient pathway.

By working alongside the Specialist Liaison Practitioner, you will work with inpatient colleagues, to support and encourage a high standard of healthcare being provided for all service users with a Learning Disability and/or Autism who also have a mental health need and are under the care of LPFT inpatient services.

It is a county-wide, 7 day per week service which functions as part of a multi-agency pathway, working with service users, providers, carers, families, social care, and health partners to enable people with a learning disability and/or autism in Lincolnshire to receive appropriate care in the right place at the right time.

LDA Enhanced Support & Liaison Service offers the following levels of support:

Level 1 – Generic support

Level 2 – Patient Specific Support

Level 3 – Enhanced Patient Specific

Within the levels of intervention and support offered there are different pathways that an individual might access. These include:

  • Community Liaison Pathways
  • Inpatient/discharge Pathway
  • Enhanced and intensive community Intervention & Support

It is the inpatient pathway that you will be focused on.

Main duties of the job

You will work across service boundaries that will include but not be limited to inpatient colleagues, community colleagues, carers and family and the individual. You will support with advice, reasonable adjustments, encouraging a collaborative streamlined approach to care planning, proactive discharge planning and readmission avoidance planning.

Under the guidance of the Specialist Liaison Practitioner, you will oversee those inpatients with a Learning Disability and/or Autism and bring in colleagues from the ESLS MDT as required. You will be working closely with mainstream services and stakeholders across the wider Lincolnshire System, encouraging a joined-up approach to the care received.
