An exciting opportunity has arisen for a committed, enthusiastic and versatile individual to work within the Unplanned Care, D2A team within the South West of Staffordshire. This fast-paced role will promote and support the discharge of individuals out of hospital and provide daily multi –disciplinary working, with a range of professionals in the social care, health, community and private sector. The service predominantly supports individuals over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.
For this role, it would be expected that you have a good experience of completing strengths based and evidence-based assessments in line with The Care Act 2014, Mental Capacity Assessments and have knowledge and experience of Safeguarding Vulnerable Adults. It would also be expected for you to have knowledge of Continued Health Care Assessments and Funding
Main duties of the job
Working in a supportive team, you would be expected to complete assessments within agreed timescales to meet the increased demand of discharges from the acute hospitals and be flexible to your working role. You will be predominantly working within the community, completing visits within local care homes and be involved with regular multi-disciplinary team meetings to promote and advocate for the individuals and their families and carers.

Social Work Learning Academy (SWLA)
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA ‘stage’ (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

- Experience of working with Adults and older Adults with mental health needs and their carers

- Experience of social work in other user groups/settings i.e. Children and Families, Learning Disability, Mental Health.
QUALIFICATIONS & TRAINING
Essential
- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
- Ensuring current registration with the Professional Social Work regulatory body at all times.
- An ongoing commitment to personal and professional development.
- To undergo appropriate training to achieve Assessed Year in Practice (ASYE)
- Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
