Locally Employed Doctor (ST3+) Anaesthetics (Simulation Fellow) Fixed term for up to 12 months

We have posts available across York and Scarborough. The clinical duties of this role include providing a comprehensive anaesthetic and critical care service to patients, primarily based in Scarborough.

The postholder must comply at all times with the General Medical Council’s Duties of a Doctor, maintaining the highest standards of professional conduct and patient care. Strict patient confidentiality must be upheld at all times in accordance with Trust policies and information governance requirements.

The postholder will undertake duties as set out in the job description, in agreement with the Clinical Director, and will be responsible for the safe and effective clinical management of inpatients and outpatients under their care. They will also be managerially accountable for the appropriate and efficient use of Trust resources and professionally accountable to the Medical Director.

On commencement, the postholder is required to attend the Trust’s Corporate Induction and participate fully in all statutory and mandatory training programmes. Full compliance with Trust policies and procedures is expected throughout the duration of employment.

The postholder must also ensure that all intellectual property rights of the Trust are respected and will work collaboratively within multidisciplinary teams to support the delivery of high-quality, patient-centred care.

Main duties of the job

The duties of the post include the provision of Critical Care & Anaesthetic services at York Hospital, delivered in line with Trust policies & clinical standards. The postholder will undertake pre-operative assessment, optimisation & pre-medication of patients undergoing elective & emergency surgery, ensuring safe & effective preparation for anaesthesia & operative intervention.

The role includes administration of local & general anaesthesia across a range of specialties, with responsibility for intraoperative monitoring & management of complications. The postholder will also contribute to post-operative care, working collaboratively with surgical teams to assess & manage complications and support patient recovery.

A significant component of the role involves providing anaesthetic services within the Obstetric Department, including elective & emergency neuraxial & general anaesthesia. The postholder will also support the Intensive Care Unit & Critical Care Outreach Team in the management of critically ill patients.

The postholder will participate in a 1:8 resident on-call rota covering theatres, ICU or labour ward, and contribute to the cardiac arrest team. Cross-site working across York, Scarborough and Bridlington.

The role includes participation in audit, clinical governance, teaching, and continuous professional development. The postholder will deputise for colleagues where necessary and undertake additional duties in emergencies to ensure continuity of patient care.

Detailed job description and main responsibilities

THE SIMULATION POST

York and Scarborough Hospitals have an active simulation program both within the anaesthetic department and throughout the wider hospital. In York there is a dedicated anaesthetic simulation room located in the theatre complex complete with high fidelity 3G Laerdal simulation manikin.

The successful applicant would be expected to contribute to the planning and delivery of simulation sessions alongside the Simulation Leads and College Tutors. This would take place cross site in both York and Scarborough Hospitals.

Examples of current simulation programmes for participation:

• Monthly Consultant sessions in General Anaesthetic, Paediatric and Obstetric Emergencies
• CRUMPET course (four times/year)-MDT simulation care of the critically ill child including ED and Paediatrics
• Resident teaching programme; including novice introductory simulation course and critical incident course
• MDT training of theatre staff (PACU nurses, ODPs, scrub nurses, TSWs) in Clinical Governance sessions

Within the anaesthetic department there are also other postgraduate teaching opportunities outside of simulation including organising and running weekly resident teaching sessions on anaesthetic and ICU topics as well as undergraduate teaching.

There would also be an opportunity to collaborate with the Trust Simulation Lead in delivering postgraduate sessions to the wider hospital audience.

The intended working pattern would be 3 days direct clinical care and 2 days in an educational role (plus the on-call commitments described above).

On appointment, discussion about enrolling in the PGMCE at Hull York Medical School would be possible.

The Anaesthetic Department is part of the Theatres, Anaesthetics and Critical Care Directorate.

At York Hospital there are 12 operating theatres in our main suite (including one dedicated to vascular imaging assisted work). We have a dedicated day unit with 6 theatres and an ophthalmology operating area with two theatres.  These are supported by two Post-Anaesthetic Care Units (PACUs).  There are also two separate obstetric theatres.

Surgical services currently supported include general surgery (including colorectal and upper GI), orthopaedics and trauma, obstetrics and gynaecology, ENT, maxillo-facial, oral surgery, ophthalmology, urology and vascular. Enhanced recovery programmes are well advanced in colorectal and orthopaedic surgery and are being rolled out in other specialties. Preoperative assessment sessions are provided as well as a full nurse-led pre-assessment service.  We also have a well established and highly regarded perioperative programme running alongside our successful CPET service.

We have a 22 bedded Critical Care Unit on the York site

Scarborough Hospital

The Anaesthetic Department comprises of a 6 bedded Intensive Care Unit, a main theatre suite of six theatres, and separate maternity and ophthalmology theatres.

Surgical services currently supported include general surgery (including colorectal), orthopaedics and trauma, obstetrics and gynaecology, ENT, dental, oral surgery, ophthalmology and urology. Enhanced recovery programmes are well advanced in orthopaedics and are being rolled out in other specialties.

Preoperative assessment sessions are provided as well as a full nurse-led pre- assessment service.

The labour ward delivers approximately 1500 babies per year with a 19% caesarean section rate. An on-demand epidural service is provided.

Additionally, anaesthetic services are provided in Bridlington Hospital, approximately 20 miles from Scarborough. The post-holder may be required to work in Bridlington as well.
