Shift pattern - 8am to 8pm, 5 over 7 including public holidays
This is an exciting opportunity for an enthusiastic and motivated advanced nurse practitioner to be pivotal to the continued implementation and development of Renfrewshire’s Hospital at Home Service. The candidate must demonstrate proven leadership skills and evidence of consolidation of advanced practice learning by means of experience and depth of knowledge evidenced by continuous development across the 4 pillars of advanced practice. The candidate must be registered with NMC and have achieved advanced practice qualification which is annotated on SWISS register. Significant experience as a practicing ANP would be advantageous for applying for this role.
The Renfrewshire hospital at home service is an integrated service with front door frailty teams within the RAH and is being developed in collaboration with East Renfrewshire HSCP. Patients admitted to the service are older adults living with frailty who present with an acute change to their baseline that requires acute level interventions which can be delivered at home.
The service over the next few months is preparing to expand to increase the bed model to 18 beds delivered over 7 days inclusive of public holidays. The continuing development of the service will require the post holder to demonstrate strong leadership skills together with a high level of clinical experience to enable them to the lead and develop the team accordingly.
This role is clinical and will require a significant proportion of the lead ANP time to be clinical, as well as working alongside service manager and key stakeholders related to operational pathway development and refinement to ensure robust clinical governance is in place.
