Stockport Foundation Trust is seeking a highly motivated, experienced Advanced Clinical Practitioner who is able to offer strong operational and clinical leadership to our team of  ACPs within the Directorate of Medicine for Older People

The post holder must have a breadth of experience and be able to demonstrate experience within the 4 pillars of advanced practice and have successfully led strategy and high performing teams.

The post holder will lead the team alongside supporting the Divisional and Trust vision for Advanced Practice.

Main duties of the job

Key responsibilities may include:

  • Leading the operational delivery of Older People's Services.
  • Ensuring high standards of patient care, safety and experience.
  • Driving quality improvement, governance and service redesign programmes.
  • Supporting patient flow, discharge planning and capacity management.
  • Leading and developing the nursing and multidisciplinary workforce.
  • Monitoring performance against key quality and operational indicators.
  • Managing risks, incidents and complaints within the service area.
  • Supporting delivery of Trust and Directorate strategic objectives.
  • Working in partnership with internal and external stakeholders to improve integrated care pathways.
  • Providing senior leadership, expert advice and professional support to the ACP team and across the Directorate.
  • The post holder will provide strategic, professional and operational leadership, ensuring robust governance arrangements are in place to support the delivery of safe, effective and responsive services for older people, while driving continuous improvement in quality, patient experience and clinical outcomes.

Detailed job description and main responsibilities

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Essential

  • Senior experience within Acute medicine / Frailty services
  • Evidence of practice development and innovation
  • Proven track record in leadership
  • Ability to undertake and utilise research findings in clinical setting

Experience

Essential

  • Extensive post registration relevant experience demonstrating the appropriate competencies for the role
  • Evidence of research/audit
  • Extensive Acute medicine / Frailty experience
  • Proven track record of involvement service redesign and change management
  • Senior experience delivering presentations in Advanced Practice

Desirable

  • Involvement with local and national committees and have strong links with local Universities

  • Must be able to work autonomously within area of expertise
  • Ability to manage a complex workload
  • Ability to prioritise and deal with complex issues
  • Direct professional development of self and others, able to supervise ACP’s and trainee ACP’s within the practice area
  • Deliver work based education to both the ACP;s trainee ACP’s
  • Able to work under pressure
  • Effective high level communication, facilitation and counselling skills
  • Self-motivated and the ability to motivate others

Education & Qualification

Essential

  • Registered Nurse/AHP with Professional Registration
  • MSc level in Advanced Clinical Practice
  • Non-medical prescriber
  • Educational Qualification or equivalent experience
  • Leadership/Management qualification

Desirable

  • Doctoral degree or willing to work towards
