Main duties of the job

Main duties and responsibilities

To dispense prescriptions and final check prescriptions

To carry out the distribution and stock control of medicines

To visit designated wards and departments and to liaise with medical and nursing staff in relation to the stock control and quality control of medicines

To liaise with ward staff and Pharmacists

To screen patients on admission to the Trust, contacting GPs and / or other healthcare providers to ascertain the patients medication history and advising Trust staff of identified discrepancies.

Screening patients own medicines brought into hospital for potential re-use.

To identify potential pharmaceutical issues that may arise during discharge planning and work with multidisciplinary to resolve.

To support the efficient procurement of all medicines used within the Trust and ensure that all medicines are procured in line with Trust & departmental policies & national frameworks.

To contribute to, the management of short-term supplier to-follows and longer-term medication shortages; and manage the application of these to ensure continuity of supply or sourcing of alternative treatments, escalating to the Lead Pharmacists or Pharmacy Operations Manager as appropriate.
