The Assistant Practitioner provides rehabilitation assessment and treatment for patients following stroke, supporting recovery, discharge planning and independence. The role involves managing a delegated caseload, implementing therapy programmes, working closely with multidisciplinary teams, maintaining clinical records, and contributing to service improvement and seven-day service delivery.

Detailed job description and main responsibilities

We are looking for an enthusiastic and motivated Assistant Practitioner to join our Stroke Rehabilitation Team. The successful candidate will work closely with therapists and the wider multidisciplinary team to support patients to maximise their independence and achieve their rehabilitation goals.

Key duties include:

• Delivering rehabilitation programmes for patients following stroke and other neurological conditions.
• Managing a delegated caseload and monitoring patient progress within agreed competencies.
• Supporting patients and families with rehabilitation, discharge planning and self-management strategies.
• Working collaboratively with therapists, nursing colleagues and community services to ensure seamless patient care.
• Maintaining accurate clinical records and contributing to high standards of care and governance.
• Supporting service development, quality improvement initiatives and the delivery of a seven-day rehabilitation service.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
