Head of Adult Learning Disability Specialist services.

JOB SUMMARY

The practitioner will work as a member of the Community Assessment Rehabilitation Service (CARS) and with members of the Community Learning Disability teams to provide specialist clinical assessment and time limited intervention for adults with a Learning Disability who present with behaviours that challenge and their carers in a range of settings including their own home, respite, day care, residential care and supported living.

The service is delivered 7 days per week 9am-9pm including weekends and bank holidays. They will participate in duty rotas to ensure effective delivery of the service. The practitioner will have direct clinical involvement and knowledge of a range of therapeutic interventions, working within the framework of Positive Behaviour Support.

KEY DUTIES / RESPONSIBILITIES

To be responsible for providing and monitoring of a specialist intensive support assessment and intervention service to adults with a Learning Disability who fulfil the criteria for access to service. This is likely to occur in day care settings, service user’s homes, and other relevant services.

To prioritise with direction from the team leader within the service, the allocation of service users referred to the team in line with individual needs for responsive/ time limited intensive support.

To support other members of the Team working with individuals on behavioural aspects of conducting assessments and implementing interventions.

Complete/ Update support plans/ risk assessments where required

To support community services, parents and carers in the provision of intensive support assessments and interventions to individuals in receipt of services from the Team

To work closely with other staff in Health and Social Services and community services to support adults with a learning disability requiring a period of intensive support.

To work collaboratively with other service providers across Health and Social Services, voluntary and private sectors in the best interest of the individual.

To contribute, in association with other professionals in the service, to the organisation and development of specialist services for Adults who have a learning disability, who require time-limited support.

To maintain and provide to the team leader all statistical data and information required by Trusts and Commissioners for service evaluation/ review and further service delivery.

To maintain appropriate clinical and other agency records as required.

To meet regularly with the clinical and operational supervisor for supervision, to ensure good standards of practice and promote career development to ensure access to relevant professional supervision.

To conform to the expectations of KSF and PD by meeting with immediate line manager in order to ensure productive functioning.

To comply with policies and procedures of the Trust.

To effectively manage a caseload.

To participate and support in the education and training of other health and social care staff in relation to the management of challenging behaviour.

To participate in the training and supervision of other behaviour specialist and staff within the wider community service.

Hold a professional Social Work Qualification and be registered with NISCC

Hold a relevant nursing qualification and be a Registered Nurse on Part 1 of the live NMC Register

Hold an Allied Health Professional qualification and be registered with the Health Professional Council

Hold a Psychology honours degree 2:1 or above plus a relevant post graduate qualification at diploma level or above and be registered with the British Psychological society.

Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.
