Job overview
Ready to play a key leadership role in the future of neighbourhood health and care?
Across Cornwall and the Isles of Scilly, health and care organisations are strengthening neighbourhood-based ways of working to deliver more joined-up, person-centred care closer to home. As we continue to deliver the ambitions of the NHS 10-Year Plan and develop integrated neighbourhood teams, this is an exciting opportunity to help shape how Adult Community Services contribute to that vision.
We are seeking ambitious, compassionate and innovative leaders to join us as Integrated Neighbourhood Community Leads. These are senior leadership roles within Adult Community Services, providing the opportunity to lead high-quality community services whilst working in partnership with colleagues across primary care, social care, mental health services, acute services and the voluntary sector.
As an Integrated Neighbourhood Community Lead, you will provide senior clinical, operational and professional leadership for Adult Community Services within a defined neighbourhood footprint. Working alongside other neighbourhood leaders and system partners, you will help strengthen integrated ways of working, improve outcomes for local people and ensure services are responsive to the needs of the communities they serve.
The role combines professional leadership, service management and partnership working, with responsibility for the quality, performance, workforce and operational delivery of Adult Community Services within your neighbourhood. You will support multidisciplinary teams to deliver excellent care, contribute to the development of neighbourhood models of care and help ensure services remain safe, effective and person-centred.
This is an exciting opportunity to influence how community services evolve within integrated neighbourhoods, helping to improve patient and staff experience, reduce health inequalities and support people to live healthier, more independent lives.
We are recruiting to a number of opportunities across Cornwall and the Isles of Scilly through a single recruitment process.
If you are passionate about community services, collaborative leadership and improving outcomes for local people, this is an opportunity to make a meaningful contribution at a time of significant change and development across neighbourhood health and care.
Successful applicants will be invited to attend an Assessment Centre on 6 November.
Main duties of the job
This is a highly visible leadership role within Adult Community Services, requiring a leader who can build strong relationships, inspire teams and work collaboratively across organisational boundaries.
Working closely with colleagues from primary care, social care, mental health services, acute services, the voluntary sector and local communities, you will represent Adult Community Services within neighbourhood partnerships and contribute to the development of integrated models of care that improve outcomes and experiences for local people.
You will provide leadership to multidisciplinary community teams, creating the conditions for high-quality, compassionate and effective care to flourish. You will champion innovation, quality improvement and continuous learning whilst ensuring services deliver against quality, performance and workforce objectives.
The successful post holder will:
- Provide senior clinical, operational and professional leadership for Adult Community Services within their neighbourhood.
- Lead and support multidisciplinary teams to deliver safe, effective and person-centred care.
- Work collaboratively with neighbourhood partners to strengthen integrated ways of working and improve service coordination.
- Contribute to the development and delivery of neighbourhood-based models of care.
- Promote innovation, continuous improvement and service transformation.
- Support improvements in population health outcomes and help address health inequalities.
- Ensure services meet quality, governance, workforce and performance standards.
- Act as a visible, accessible and compassionate leader for staff and stakeholders.
The role requires a leader who can balance operational delivery with longer-term service development, ensuring Adult Community Services play a key role within wider neighbourhood partnerships. Whilst accountable for community services within their portfolio, the post holder will work collectively with other neighbourhood leaders and system partners to support the shared ambition of delivering joined-up, effective and sustainable care closer to home.
In undertaking the duties of this post, the post holder must comply with the NMC/HCPC Code.
Where applicable, the post holder will be responsible for ensuring services within their portfolio maintain compliance with applicable Care Quality Commission (CQC) standards and regulatory requirements, supporting the organisation in meeting its statutory responsibilities.
