Main duties of the job

The pharmacy technician will:

  • Conduct domiciliary visits to patients in Westminster and Kensington & Chelsea boroughs to provide a medicines optimisation service
  • Conduct medicine reconciliation
  • Assess medicines related needs, support adherence and non-compliance, and facilitate implementation of safe handling and administration of medicines
  • Demonstrate collaborative working with the wider health and social care multidisciplinary team to reduce medication related risk to patients in their own homes
  • Participate in the delivery of medicines optimisation initiatives
  • Participate in audits within the Trust

Detailed job description and main responsibilities
