Job summary

An exciting secondment position has arisen to join the Inflammatory Bowel Disease (IBD) Nursing Team as a Band 6 Clinical Nurse Specialist. They will be an integral member of the IBD team, which provides care and treatment for patients with inflammatory bowel disease in an inpatient and out patient setting. This is a development opportunity to develop your expertise in specialist IBD care, working closely with IBD team and wider multidisciplinary team. Youll play an important role in supporting patients through diagnosis, treatment, flare management and ongoing disease monitoring, helping them to understand and take an active role in managing their condition. Youll have the opportunity to develop advanced clinical knowledge, including assessment, treatment pathways, medicines management, biologic and advanced therapies, patient education and telephone or virtual clinical support. Youll use your clinical judgement to recognise deterioration and disease activity, coordinate appropriate care and provide timely advice and support.

It is essential for the post holder to develop strong professional relationships with colleagues, Trust employees, and staff from external agencies.

The IBD nurse specialist will be flexible to meet the needs of the team and will demonstrate knowledge, expertise and passion in working within the speciality of Inflammatory bowel disease.

Main duties of the job

To assess and support patients with IBD, including recognising symptoms of active disease, complications and deterioration. To provide specialist advice and education to help patients understand and manage their condition.

To support patients receiving biological and advanced therapies, including treatment monitoring, safety checks and ongoing review.

Responding to patient concerns and providing inpatient support, out patient clinics, telephone, virtual and face-to-face advice, helping patients access the right care at the right time.

Work collaboratively with gastroenterologists, surgeons, dietitians, pharmacists and other members of the multidisciplinary team to coordinate holistic care.

Supporting patients through flare-ups, investigations, treatment changes and periods of remission. Monitoring treatment response and contributing to clinical assessment, care planning and disease monitoring. Recognising when patients need escalation and ensuring concerns are communicated promptly to the appropriate clinician or service.

Contributing to clinical audits, research, quality improvement and service development, helping us continually improve IBD care.

To provide a continuously improving service in line with service requirements.

To communicate effectively with all members of the multi-disciplinary team, patients, carers and the public.

Job responsibilities

To assess, monitor and review ongoing patient care. Interpreting information and using specialist knowledge and judgement in the initiation and escalation of medical treatment.

To provide clinical advice, teaching and expertise to other nursing staff including Primary and Secondary Care, allied professionals, patients, families and carers.

To support in the service development of inflammatory bowel disease in line with evidence based practice and Clinical Governance requirements.

To be prepared to undertake further training/develop clinical skills in line with necessary service developments.
