Job summary

We have a very exciting opportunity within the Wakefield Intensive Home Based Treatment Team as we have a vacancy for a In reach Mental Health Practitioner at band 6, this is a new role within the team. The team currently consists of Social Workers, Nurses, Medical Practitioners, Psychologists, Trauma Informed Personality Disorder Practitioner and a Non Medical Prescriber, Dual Diagnosis Practitioner, as well as Health Care Support workers and Admin staff.

The team operates within the whole system of adult mental health services, and is the single referral point for people in crisis, offering assessment and the provision of intensive home-based treatment for those for whom hospital admission would otherwise be necessary.

The service operates 24 hours a day 365 days a year. The role will involve working flexible hours to ensure that all ward rounds and business meetings are attended providing a smooth transition between inpatient admission and returning home.

You will be committed to team working, have excellent practice skills and the ability to work flexibly and creatively to meet the needs of service users and their carers. You will also be required to work autonomously with the support of the MDT.

Main duties of the job

The successful candidate will be expected to lone work, carrying out home visits, attend ward rounds and meetings related to the wards workings, carry out assessments and triage as well as supporting their colleagues with patient contacts that may require more than one practitioner. There would be an expectation for documentation to be carried out to a high standard with the opportunity to gain knowledge on the wider services available in the Wakefield area.

We offer regular supervision which includes team supervision as well as individual supervision, we actively encourage personal development through interagency training and any other CPD you may find of benefit to your practice.

Job responsibilities

JOB SUMMARY

The role of the CRS practitioner is to provide screening, triage, comprehensive assessment of mental health and social care needs, formulation of plans of care, and where indicated to deliver home based treatment to people with a severe mental illness who present in crisis or who present a significant and immediate risk of harm to self and others.

CRS practitioners include mental health nurses, approved social workers, social workers and occupational therapists.

KEY RESULT AREAS

1 Provides a service that is integral to the whole systems model established within the Wakefield locality.

2 Provides a service that is congruent with South West Yorkshire Mental Health NHS Trust Crisis Resolution Service aims i.e. service user focused, accessible and responsive on a 24 hour, 365 day a year basis ensuring that triage, assessments and intensive community care are provided to service users who require crisis resolution.

3 Acts as primary gatekeeper of admissions to adult acute inpatient units.

4 Works within the care programme approach.

5 Screens referrals to the Crisis Resolution Service, to determine suitability of service provision and priority for service response for people who are experiencing a crisis with a severe mental illness or presenting with significant and immediate risk of self-harm or suicide, or presenting with risk to others, or presenting as potentially requiring in-patient care.

6 Effectively triages all referrals, gathers information from and where necessary advises referring medical and health and social care staff, and in collaboration with the service user and carers and other professionals determines the appropriate clinical pathway for the service user.

7 Where indicated provides comprehensive evidence-based assessment of the health and social care needs of service users and their carers presenting in crisis in a variety of settings including general hospital departments, GP surgeries and in service users homes.

8 Assesses risk in line with locally agreed protocols.

9 Following assessment determines appropriate outcomes and implements plans of care which may include the provision of home based treatment or may include facilitating admission to hospital; the instigation of assessment under the Mental Health Act 1983; the provision of advice to service users and their carers; referral to other services; advice to other professionals including GPs and general hospital staff on management of care;

10 Effectively liaises with primary and secondary care providers and other agencies to signpost and develop pathways of care for service users and their carers.

11 Conveys the outcome, plan and any recommendations from the assessment to the referrer and any other agencies involved in the service users care in the form of written documentation.

12 Provides comprehensive ongoing assessment of those service users accepted for home based treatment from the Crisis Resolution Service as an alternative to inpatient care. Undertakes ongoing assessment of risk and ascertains the service users mental health and social care needs. In collaboration with the service user and their carer, and any existing agencies involved, formulate and implement a specialised crisis plan of care incorporating the management of risk.

13 Maintains an holistic approach to care considering evidence-based approaches and carries out a range of specialised evidence-based interventions with those service users receiving home based treatment to work towards resolution of crises, promoting recovery, social inclusion and the realisation of coping strategies.

14 When delivering home based treatment, ensures that assessment of mental health and risk continues as an ongoing process, that a team approach is taken in planning and evaluating care including participation in medical and clinical reviews and joint working with existing care teams.

15 Undertakes a care co-ordination role for individual service users as a part of the Crisis Resolution Service caseload, including the implementation of care plans and preparation of comprehensive documentation in relation to service users, performing reviews and arranging for appropriate transfer/discharge procedures to take place in line with Care Programme Approach and in collaboration with relevant professionals and case managers.

16 Provides information, education and support for service users, family members, carers and other professionals. Offers formal and specific mental health education where appropriate for the maintenance of optimum mental health and prevention of relapse. Formulates a relapse signature and effective response plan in conjunction with the service user, carer and care team

17 Provides appropriate information to other members of the multi-disciplinary team and relevant care providers to ensure that the programme of care established for the service user and their carer continues when others provide care.

18 Facilitates timely discharge from hospital for those service users for whom home based treatment can provide an alternative to continued inpatient care

19 Where approved under the Mental Health Act 1983, carries out duties as an Approved Social Worker in accordance with statutory and agency requirements and in line with locally agreed procedures and protocols.

20 Works within appropriate agency frameworks, policies and guidelines in relation to the management storage and administration of medication, including administration of medication by injection where appropriately qualified.

21 Where appropriately qualified, acts as a supplementary and extended prescriber in line with legislative frameworks and trust guidelines.

22 Where appropriately qualified, undertakes physical health assessments including TPR and BP and, after appropriate training, phlebotomy.

23 Operates in a range of community and agency environments, including general hospital settings, bearing in mind the relevant policies and procedures for those settings.

24 Promotes service user and carer participation in the service and works in partnership to formulate and deliver plans of care.

25 When acting as triage practitioner, is responsible for assessing coordinating and prioritising resources including the allocation and prioritisation of tasks and deployment of staff.

26 Provides specialist services, education and consultation to acute mental health services and other health and social care professionals with regards to assessment, care and management of acute mental illness or presentations of suicide risk or risk to others.

27 Maintains an effective working knowledge of statutory guidance and legal frameworks relevant to crisis resolution.

28 In conjunction with the other CRS practitioners, undertakes the supervision of junior practitioners and students contributing towards specific professional development programmes.

29 In conjunction with other CRS practitioners, undertakes the supervision of the work of Mental Health Support Workers with service users on home based treatment.

30 Represents the service and promotes a positive public image of CRS with service users and carers, other agencies and throughout the organisation.

31 Contributes to effective team working and to the promotion of positive team morale.

32 Undertakes any other tasks commensurate with this grade and role or other duties as required following consultation with line management.
