Job overview

The Integrated Care Coordination Centre (ICC) is an innovative joint initiative between the London Ambulance Service (LAS), ICBs, Acute and Community Providers and key healthcare partners. It offers clinical advice by drawing on expertise from Emergency Medicine and other hospital Consultants, GPs, LAS clinicians, Mental Health Services, Primary Care, and Urgent Community Response (UCR) teams. The aim of the service is to support clinicians in making informed decisions regarding patient conveyance, alternative care pathways, or safe non-conveyance options, and to navigate patients directly to the most appropriate part of the Urgent and Emergency Care (UEC) system.

Main duties of the job

  • Support the smooth and successful running of the ICC Hub
  • Facilitate case flow within the ICC hub by overseeing the case queue and supporting case allocation amongst ICC clinicians
  • Effective telephone communication with clinicians from across the wider healthcare system
  • Accurate and timely data entry and record keeping, including validating data entered by colleagues
  • General administrative support within the ICC Hub
  • Supporting the ICC management team in quality improvement work within the ICC hub

Detailed job description and main responsibilities

This strategic, multi-agency approach ensures patient-centred care, and reduces unnecessary Emergency Department attendances by providing safe and effective alternatives. The ICC Hub Care Coordinator will play a vital role in the day-to-day operation of the ICC service, coordinating the activity of the service to ensure patients receive a timely response. They will be responsible for overseeing case flow through the hub, ensuring timely and accurate data collection to support reporting and quality improvement activity, taking phone calls from clinicians as a point of access into the service, and supporting the onward referral of patients, as directed by the senior clinical decision makers and manage the flow of requests coming through the shared ICC inbox. The coordinator will assist the clinical operations manager with hub operations, including rostering, updating internal and external rostering systems, tracking and reporting clinical incidents via Radar, and managing LAS staff communications as directed. The ICC Hub Care Coordinator will support the smooth running of the ICC service and support colleagues with data entry and administrative tasks that can ensure service efficiency
