Job summary
Nottingham CityCare is looking for a passionate, committed and driven Assistant Practitioner to join the Homeless Health Team.
The award-winning Homeless Health Team provide outreach healthcare and advice in Nottingham City to individuals experiencing Severe and Multiple Disadvantage (SMD).
We are looking for candidates with excellent, adaptable interpersonal skills who can work independently and as part of a team who share CityCares values, behaviours and vision of providing excellent care in the community.
This is a small, hardworking and innovative team consisting of specialist homeless nurses and a care coordinator. This new post as an assistant practitioner will involve working alongside the team delivering nurse led interventions to service users.
Interviews will take place on the 16th October 2026
Main duties of the job
The successful candidate will work alongside and support the Specialist Nurses and Care Coordinator within the Homeless Team to identify, assess and provide treatment interventions for people in Nottingham City who are experiencing homelessness and severe multiple disadvantage, and who require health and social care support.
The role will involve developing and maintaining effective working relationships with stakeholders across the severe multiple disadvantage partnership, supporting collaborative care and helping to improve patient outcomes.
The post holder will maintain accurate records and work as an autonomous practitioner, with appropriate supervision, across professional boundaries and in line with relevant legislation, policies, procedures and organisational values.

Job responsibilities
Job Purpose
To work alongside and support the Specialist Nurses and Care Coordinator in the Homeless Team to identify, provide assessment and treatment intervention to people within the Nottingham City who are experiencing severe multiple disadvantages (SMD) who are homeless and require health and social care intervention.
To encourage and build working relationships with stakeholders within the SMD partnership to provide collaborative care and improve patient outcomes.
To maintain records as an autonomous practitioner working with supervision across professional boundaries and within legislation, policies, procedures and core values.
Dimensions
Work in conjunction with and under the supervision of Homeless Health Specialist Nurses.
The post holder will provide assessment and treatment intervention as directed by the Homeless Health Specialist Nurses.
Signpost on behalf of the Homeless Health Team, giving advice and guidance on processes to health and social care professionals/statutory voluntary sector within the SMD partnership stakeholders.
The post holder will work within Nottingham Citys Homeless Health Team managing information, requests and referrals.
The post holder will work collaboratively with partner agencies to include NUH, Notts Healthcare Trust, Prison & Probations service and Changing Futures.
Able to fulfil physical demands of role, including manual handling applications.
Will be required to travel city-wide for visits/meetings.
Key Responsibilities
Clinical
To undertake individual treatment and interventions with clients under the supervision or direction of qualified staff to promote patient well-being and safety.
The post holder requires competency or a willingness to complete further training in clinical skills such as venepuncture, clinical observations and breakaway training.
Within competency recognise abnormal readings and changes in patients physical, mental wellbeing, or cognitive condition, escalating to registered nurse or clinician for advice.
To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner role.
To attend and contribute to team meetings and case discussions using effective communication. To participate in clinical discussions/meetings regarding patients care collaboratively with stakeholders in the SMD partnership.
To maintain comprehensive and accurate records in line with employers polices
To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations. To dynamically assess risk to maintain personal and patient safety.
To have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties.
To actively promote equality and diversity within service delivery and peoples individual rights.

Work in an agile way to respond and meet the health requirements of the homeless population with service partners or a specialist homeless health nurse.
Care Navigation
To assist the Homeless Health Team to manage the caseload.
To make judgments regarding referrals, using triage protocols to ensure that citizens are seen by the most appropriate team member at the right time to meet their needs. Whilst recognizing the need to involve, or seek advice from, more experienced colleagues as necessary for clinical decision making.
To be responsible for the co-ordination and to liaise with all relevant statutory and voluntary sector services responsible for the patients care to arrange the necessary support.
To be responsible for a safe hospital discharge for citizens through health and social care to ensure a smooth handover to a named service.
To identify information gaps resulting in interventions much earlier in the patients journey to prevent a relapse of their condition and/or a re-admission into hospital.
To gather information and liaise with partners and other services to identify a patients location if not seen for a prolonged period.
To gather information from multiple sources to produce accurate and timely reports and patient summaries for clinicians and other care providers.
Manage referrals to the service.
Audit and add patient GP registrations to all patients that come in to contact with the Homeless Health service.
Cross reference service and hospital records for hospital attendances updating relevant parties and the patients health records where necessary.
Record and cross reference for accuracy any relevant information on health records from partner agencies for patients on the service caseload.
Undertake pilots and service developments providing administrative and clinical support.
Responsible for the procurement and audit of stock and equipment within the service. This includes the arrangement of maintenance and the recording of medical devices within the service.
To attend the SMD wraparound MDT meetings and provide updates to clinical staff within the team.
General
To support the proactive identification of citizens at high risk of admission. Be responsible for the monitoring of acute admissions and to update data recording systems and clinicians / practitioners/ other agencies where appropriate.
As a team to review, analyse and co-ordinate data to produce evaluation reports.
To input information from partners to the health record where relevant.
Ensure that health record data requirements are completed for all patients on the service caseload. This includes the entry of groups and relationships including, where applicable, key workers.
To undertake any additional duties as appropriate and delegated by the Clinical Service Manager
To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames.
To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development. Also, to keep an active up to date CPD portfolio.
