The Richmond PDITT service provides a NICE guidance-led MBT Intensive Treatment Programme and a step-down phase back to the GP, for patients primarily diagnosed with EUPD.

The post holder will hold a caseload of individual patients to maintain their core skills of MBT provision within a specialist Tier 3 Personality Disorder Intensive Treatment Service, along with co-facilitation of assigned MBT groups.

This post is open to professionals from a range of disciplines, and their core professional validation requirements will be retained.

The post holder is expected to:

• Be committed to effective and cohesive team working, and to the adherence-based delivery of the treatment model of Mentalization Based Therapy (MBT).
• To co-facilitate MBT groups and hold a small caseload of weekly patients.
• To promote and maintain a high standard of patient care.

Livingston House, Teddington

Livingston House serves as base for Richmond Adult Community Mental Health Services, close to the vibrant high street and the river Thames with a range of shops, cafés and restaurants. Livingston House is a ten minute walk from Teddington station and is served by local bus routes.

Main duties of the job

• To facilitate group and individual clinical supervision for MBT provision and modality.
• To provide highly specialised psychological interventions (MBT) to patients in Personality Disorder Teams.
• To co-facilitate MBT groups and hold a caseload of weekly patients.
• To hold a senior lead role for the Richmond CEN Referrals Pathway processes
• Provide specialist MBT Assessments for patients initially screened for MBT. Assessment processes are based upon clinical appropriateness, interpretation and integration of complex data from a variety of sources, and as required for EUPD screening set at approved Trust level. This includes clinical history taking, SCID ll, self-reporting measures (DIALOG), DERS rating scales, alongside direct clinical observations.
• To supervise and support the psychological assessment and therapy provided by other clinical members of the teams who provide psychologically informed care and treatment, and be responsible for the governance of MBT provision, and broader care planning.
• Work alongside the team, and senior clinicians within the borough, to ensure the MBT Intensive Treatment Programme is delivered, monitored and evaluated, and to take a lead on evidencing outcome measures for senior management.

Detailed job description and main responsibilities

• To provide scheduled specialist MBT clinical supervision for the team – both group and individual, to enhance containment within the team, along with providing, as required, a consultative role for teams within the service line, who may also require both guidance and containment when working with those with a diagnosis of EUPD.
• To hold a senior lead role for the Richmond CEN Referrals Pathway processes and provide specialist MBT Assessments for patients initially screened for MBT. Assessment processes are based upon clinical appropriateness, interpretation and integration of complex data from a variety of sources, and as required for EUPD screening set at approved Trust level. This includes clinical history taking, SCID ll, self-reporting measures (DIALOG), DERS rating scales, alongside direct clinical observations.
• To evaluate and make evidence-based decisions about the most clinically appropriate treatment options for those with a diagnosis of EUPD within the assessment and engagement phase, and in line with the treatment modalities provided within the Trust, along with identifying co-morbidities that might need to take precedence, or be co-worked with, holding the MBT lens, and alongside highly complex factors concerning historical and developmental processes that have shaped the individual.
• Within the MBT assessment processes, to gather information about psycho-pharmacological treatment and to liaise with IRH medical colleagues to formulate a plan to review and manage psychotropic medication whilst the patient is in the assessment and engagement phase. Medical reviews are then led by the PDITT consultant once the patient is under the sole care of PDITT.
• To play a key role in the twice daily team Zoning Meetings and in the weekly Team Meeting which focuses on clinical work, governance, and Trust wide/service needs business items.
• To formulate and implement care plans for the formal treatment of MBT and the management of a patient’s presenting mental health needs and identifiable social care needs, with full involvement with the patient. This includes MBT Formulations (known as ‘My Relational Passport’), Genograms, and Collaborative Crisis and Contingency Planning.
