This role offers an exciting opportunity to further develop the Enfield Proactive Care Service Multi-Disciplinary Team (MDT) model and contribute to the design and delivery of an exemplary Integrated Neighbourhood Team (INT)

To deliver a high standard of Occupational Therapy care to patients/clients and their families/carers within a community setting, ensuring services are responsive to the needs of a diverse and multicultural population.

To work alongside senior colleagues in providing specialist Occupational Therapy expertise and advice to the wider service and multidisciplinary teams.

To ensure that all Occupational Therapy assessments are completed within agreed service timescales and quality standards.

To be responsible for the day-to-day coordination and delivery of Occupational Therapy interventions within the service, managing a designated caseload and providing care in accordance with service specifications.

To support, educate and empower patients and their carers to manage complex rehabilitation and Occupational Therapy needs, enabling them to maximise independence and effectively manage their condition.

• The Post Holder will provide highly complex and specialist assessments and interventions which promote optimum recovery using a personalised care approach to support patients to achieve their goals, addressing physical and emotional well-being needs - ensuring that a high-quality holistic service is delivered in an area with a fast developing evidence base.
• To determine the most appropriate setting for assessment and or treatment e.g. community, , home.
• To be responsible for organising and planning workload and manging caseload to meet service and patient priorities, readjusting plans as situations develop.
• Make highly skilled evaluations and decisions about functional, cognitive, emotional well-being, motor and social recovery with a focus on fatigue management and enhanced coping strategies.
• Use evidence-based practice and client centred principles to assess, plan, implement and evaluate interventions.
• To demonstrate advanced clinical reasoning skills, experience and knowledge of evidence-based practice to determine treatment plan using a wide range of treatment skills and options.
• To be able to identify own professional limitations and seek professional advice when necessary.
• To undertake responsibility and leadership for completion and implementation of clinical projects, audit or research.
