An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Service based in Chester.

This service provides specialist assessment, treatment, interventions & support, to help people recover from complex mental health problems & to regain the skills & confidence to live successfully in the community.

The role of Healthy Lifestyle Coach within the team will encompass working within the physical health pathway with patients within the community to promote healthy lifestyles to support them to achieve their goals.

Main duties of the job

As a Healthy Lifestyle Coach you will be working alongside patients in the community rehabilitation team to assess their lifestyles and wellbeing, set goals for improving their health, agree action-plans, and provide practical support and information that will help people to change their behaviour. This will include promoting the benefits of being physically active and eating healthily, reducing alcohol intake, breastfeeding, practising safe sex, stopping smoking, etc.

The post holder will help people identify how their behaviours may be affecting their health and support individuals  to create a health plan.

This role will involve taking the lead in planning, organising and running the physical health clinic. This will include monitoring of parameters (e.g. BP, weight, height, BMI)bloods, ECGs in clinic and follow  up of outcomes and liaising with primary care and medical specialists as appropriate.
