Job summary

Join a dedicated team providing health care support for patients' in their homes across Plymouth, South Hams, and West Devon. This part time role (up to 29.5 hours per week, is ideal for an experienced and motivated health care assistant eager to contribute to a growing Urgent Care Nursing service.

You will deliver high-quality, patient-centered care in the community, helping individuals remain safe and independent at home while supporting their health and wellbeing. The role involves utilising clinical skills and expertise within the Urgent Care Nursing Service.

We seek a health care assistant with good clinical knowledge.

Supporting the multi disciplinary team using clinical skills such as manual observations using NEWS 2, venepuncture and ECG's.

You will be required to deliver and set up virtual monitoring equipment in patient homes (you will undertake training for this) deliver and collect equipment. Undertake wound care and using wound care app. Administer sub cutaneous mediation as prescribed once trained and competent.

Main duties of the job

The successful candidate will be responsible for the delivery of high quality care for patients within their homes.

Liaise with the nurse in charge and referral coordinator within the Urgent care nursing service team, and will be supported by senior clinicians & clinical manager.

Responsible for effectively assessing, monitoring & escalating concerns regarding patient care using SBAR tool.

Supporting the multi disciplinary team with care planning using clinical skills such as manual observations using NEWS 2, venepuncture and ECG's. You will be required to deliver and set up virtual monitoring equipment in patient homes (you will undertake training for this) deliver and collect equipment. Undertake wound care and using wound care app. Administer sub cutaneous mediation as prescribed once trained and competent.

The Urgent Care Nursing team work on a flexible rota shift system.

The service covers 24 hours per day over seven days a week. However the Acute Virtual Ward health care assistant role runs from 08:00 - 17:00, however the finish time can be negotiated but finish time no earlier than 16:00.

The ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations, including areas not served by public transport, is essential.

Job responsibilities

SCOPE AND RANGE

To deliver and effective and competent level of HCA support and consistently deliver a patient focused provision which promotes good customer service and effective working relationships.

To work without direct supervision within the clearly defined boundaries of your competencies and carry out specific delegated clinical tasks and responsibilities, prioritising work in line with policies and procedures.

To take a holistic approach to patient management, carry out tasks and treatment plans as directed by registered staff, ensuring to report back to the appropriate worker any changes in clinical conditions and maintain accurate clinical records.

To participate in all mandatory training required for the role and participate in training that is role specific which includes training to respond to urgent situations e.g. safeguarding, untoward incidents.

Within your job role there will be clinical- specific competencies which you may need to undertake to enhance your knowledge and skills in practice. this may vary across clinical areas.

To work within Livewell Southwest policies, maintaining flexibility to carry out other duties as may be reasonably required and the ability to move across the organisation should the need arise for example in a pandemic situation.

To provide clinical and clerical duties relevant to the service you work within.

You may be required to travel and attend to service users based across Plymouth and other geographical areas.

You may be required to undertake 7 days working, including bank holidays, this is subject to the terms and conditions set out in your contract of employment.

MAIN DUTIES/RESPONSIBILITIES

Responsibility for People Management

To be responsible for contributing to the planning, monitoring, and evaluating of patient centred care. Reporting back to the registered nurse and escalating concerns as required.

Following a Registered Nurse or Assistant Practitioners assessment of need the post holder will then implement a planned programme of care, with the support of a Registered Nurse.

To act as a role model for all staff and students demonstrating clinical skills and high standards of practice and professional conduct.

To take responsibility for your own personal and professional development, maintain competence, knowledge, and skills commensurate with the role.

To participate in Reflective Practice, Practice Supervision and Appraisals and line management supervision as per policy.

To take a flexible approach to supporting colleagues in times of caseload pressures, including working with colleagues to maintain support across the teams with the resources available.

Responsibility for financial and/or physical resources

You will have no direct responsibility for finances; however, you will adhere to the relevant trust policies on financial issues.

To exercise a duty of care in relation to the use of the organisations equipment and resources in a cost-effective manner.

Monitoring and ordering of stock levels and to utilise resources economically and effectively and promote cost consciousness throughout the service.

To have a working knowledge and be responsible for ensuring nurses and patients equipment is in safe working order, disposed of or returned when no longer required as per Policy and to inform Line Manager if any problems occur.

To use Livewell Southwest resources responsibly.

Responsibility for administration

To maintain accurate records, which are confidential, contemporaneous, and legible, with all care given documented at the time in line with clinical guidance in Livewell Southwest policies.

You will be expected to participate in standard of patients records by audit or peer review and have a regular open discussion with team members.

Make appropriate records in the multi-disciplinary patient file and / or electronic records, which may be required to be counter-signed by a registered nurse.

Complete on time and submit all necessary forms, documentation, including IT data and forward as required by the manager/organisation.

To always maintain and preserve confidentiality, and be aware of GDPR, Access to Health Records and Consent for Treatment.

To access NHS email and other information through Livewell Southwest intranet.

To be responsible for organising own time management daily in line with caseload demands, using own initiative and be flexible when organising own workload on a day-to-day basis, which is often unpredictable in order to respond to the needs of the service.

Where appropriate, to organise hospital records upon delivery ensuring they are available to the correct clinician. Using the appropriate clinical systems to track and trace and returning when completed.

Responsibility for people who use our services.

To provide appropriate support and advice to family and carers, demonstrating tact and sensitivity in dealing with complex and sensitive information and negotiating a positive outcome.

Act as a patient advocate as required ensuring individual needs, preferences and choices are delivered by the Service.

To ensure privacy, dignity and human rights are respected for patients and carers.

To provide basic education and continuing support to patients, family, and carers.

To identify areas of concern regarding practice and communicate to the registered nurse.

Assist registered nurses in delivering high standards of care to patients referred to Livewell Southwest services.

To recognise and respond to any changes in patient care situations, which might include risks to self and others including adult and child protection concerns. You must inform the most appropriate professional at the earliest opportunity. Be conversant with the safeguarding adult and child protection procedures and policies.

To support the delivery of holistic evidence-based/best practice care to patients in accordance with National and organisation approved policies/procedures and individual care plans.

To undertake specific clinical tasks as delegated by registered staff having once undertaken relevant training and deemed competent. Working in line with the Livewell Southwest Delegation to band 3 and 4 support workers guidance.

You will be required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager.

Responsibility for implementation of policy and/or service developments

You will be required to comply with all relevant policies and procedures pertinent to your post. Current versions can be found on Healthnet or via your manager. Failure to follow correct policies and procedures may result in disciplinary action.

To contribute to the development, implementation, and monitoring of standards of care.

Other Responsibilities

To develop and maintain own clinical expertise by attending meetings, study days and in-service training and learn new specialist skills required for service development. To maintain an up-to-date personal competency portfolio.

Only undertake work which is within own competence and to identify any work required to be undertaken by more senior staff.

To identify and take responsibility in the maintaining of clinical competencies in line with the job role. Ensuring these are adequately supported by clinical sign off. Identifying areas where this may be required.

To participate in team activities to develop and consolidate a cohesive and supportive team ensuring openness within the team.

To attend and participate in meetings as negotiated within the team and to provide relevant timely feedback.

You will be required to use a computer, being responsible for timely recording patient activities for IT data collection, dealing with e-mail queries, stock/equipment ordering.

To participate in an annual appraisal of their work in line with the Knowledge and Skills Framework (KSF) where the job description will be reviewed, and objectives set. In line with the annual development plan the postholder will be expected to undertake any training or development required to fulfil their role.

COMMUNICATIONS AND RELATIONSHIPS

To demonstrate effective communication with patients, staff, and other members of the multi-disciplinary team. When communicating with patients it should be at the appropriate level for patients to understand such as patients suffering from terminal illness, dementia, disability, visually impairment and/or hearing impairment.

Able to communicate the role and eligibility criteria of the service in which you work within.
