We are looking for a motivated individual to work as part of our multi-disciplinary team as a health improvement facilitator to focus on ageing well and proactive care in care homes.

This role will predominately work on delivering structured programs of support and proactive care to care staff and residents to improve health outcomes for residents living in the care sector. Main duties of the job

  • The post holder will work as part of the Community Care Home Support Team, supporting the delivery of education, training and evidence-based interventions to promote physical activity, falls prevention, healthy ageing and wellbeing for people living in care homes across Bristol, North Somerset and South Gloucestershire.
  • Working under the direction of registered clinicians, the post holder will support care home staff to develop the knowledge, skills and confidence required to embed preventative approaches into everyday care. This will include delivering group education sessions across a broad range of topics including physical activity and supporting quality improvement initiatives to promote best practice in ageing well.
  • The post holder will work collaboratively with multidisciplinary colleagues, care home providers and partner organisations to improve outcomes for residents by encouraging person-centred care, maintaining independence and reducing avoidable deterioration associated with frailty, inactivity and falls.
  • The role will contribute to service development through data collection, audit, evaluation and the promotion of evidence-based practice, whilst acting as a positive ambassador for the Community Care Home Support Team.

Person specification

Qualifcations and Training

Essential

  • NVQ Level 3, Foundation Degree, Assistant Practitioner qualification, or equivalent experience in health improvement, health and social care or a related field.
  • GCSE (or equivalent) in English and Mathematics (Grade C/4 or above).

Desirable

  • Evidence of continued professional development.
  • Knowledge of the Enhanced Health in Care Homes (EHCH) framework.

Skills, Experience, Knowledge

Essential

  • Experience of working in a health, social care or community setting.
  • Experience of delivering education, coaching or training to individuals or groups.
  • Evidence of managing own workload in a timely manner
  • Ability to travel independently across the locality to visit multiple care homes.
  • Competent IT skills, including Microsoft Office and willingness to learn electronic clinical systems.
  • Excellent communication and interpersonal skills, with the ability to adapt information for different audiences.

Desirable

  • Experience of working within care homes.
  • Experience of working in a multi-professional team
  • Experience of carrying out therapy under direction of AHP's
  • Understanding of safeguarding principles and responsibilities
