
We are seeking an enthusiastic Health Care Assistant (HCA) to join our team as a Band 3 HCA. In this role, you will be a valued and integral member of the Childrens Diabetes Multidisciplinary Team (MDT), undertaking delegated tasks to support highquality paediatric diabetes care. This is a rewarding opportunity to make a meaningful difference to the lives of children and young people aged 019 years living with diabetes across our locality.
We are a progressive and passionate team who continue to develop and adapt delivery of care in line with our local and National diabetes colleagues, adapting our MDT approach ensuring that we continue to deliver a service that truly changes lives and improves outcomes for our young people. This role offers a chance to grow and develop the role to continue to deliver compassion, curiosity and commitment to a role and service in making a difference.

You will work within a friendly and dynamic team comprising a Band 7 Lead Nurse, Band 6 Diabetes Nurse Specialists, and dedicated administrative colleagues. We also work closely with a wider MDT including dietitians, psychologists, and diabetesspecialist consultants.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
