
We are looking to recruit a motivated and proactive Band 3 Healthcare Assistant (HCA) to join our Primary Care Mental Health Networks (PCMHNs) across Harthill and River & Wolds.
This is an exciting opportunity to be part of a developing, community-based service supporting individuals with mild to moderate mental health needs. The role focuses on providing practical, person-centred support to help individuals improve their wellbeing, maintain independence, and engage meaningfully with services.
The successful candidate will be expected to work with a high degree of self-motivation and autonomy, managing a defined caseload of service users while working under the guidance and support of registered clinicians. You will play a key role in delivering planned interventions, monitoring progress, and supporting individuals to achieve agreed goals.
This role requires someone who is confident working independently in the community, able to prioritise their workload effectively, and comfortable making day-to-day decisions within clearly defined boundaries, escalating concerns appropriately when needed.
You will be fully integrated within both Harthill and River & Wolds PCMHN teams, attending and contributing to both MDT meetings.
The post holder will need to be flexible, organised, and able to adapt to the evolving needs of the service, while maintaining a consistent, reliable presence for the individuals they support.
The post holder will support the delivery of care plans developed by Registered Clinicians, working directly with service users in community settings.
Duties will include supporting individuals to engage with services, promoting wellbeing and independence, and assisting in the delivery of low-level interventions such as guided self-help and stabilisation work.
The role involves monitoring mental and physical wellbeing, carrying out basic observations where required, and reporting any changes or concerns to senior staff in a timely manner.
The post holder will contribute to effective communication within the multidisciplinary team, maintain accurate clinical records using System 1.
There will also be an expectation to work flexibly across the PCMHN footprint, supporting both Harthill and River & Wolds teams in line with service need.
