An exciting opportunity has arisen within the Liaison & Diversion Team for the role of Liaison & Diversion Support, Time, Recovery Worker based in *Worcestershire Outreach*

Liaison & Diversion (L&D) services identify people who have substance misuse, mental health, learning disability or other vulnerabilities when they first come onto contact with the criminal justice system. We look to identify unmet need and vulnerabilities that often lead to or exacerbate offending behaviour, offer onward referral and signposting as appropriate. We liaise across agencies to support onward planning and intervention to address identified needs and where appropriate support the diversion of individuals away from the criminal justice process into a more appropriate setting.

We work in Police Custody Suites, Courts and Community throughout *West Mercia* and offer assessments to individuals within the Criminal Justice system including Voluntary Attendees. We help to inform decision makers with relevant and timely information to support sentencing and disposal outcomes.

This is an 'all age' service and so you will be required to work with people from the age of 10 years upwards. *The post holder will be expected to travel across the MPFT footprint*

Main duties of the job

**Please refer to the attached Job Description in summary:** •    Working with service users referred to Health & Justice and the individual’s carers, helping to promote self-determination, choice and autonomy. •    To effectively establish a therapeutic relationship with service users and relatives / carer.  •    Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy. •    Assess patient's individual care needs, agree and refer on to appropriate agency.  Involve the individual in the planning and prioritisation of their activity where possible. •    Provide short term intervention to encourage individual to engage with services and attending their initial appointments.  •    Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.   •    Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions. •    Utilise agreed outcome measures to review the effectiveness of process, as part of Health & Justice Service. •    Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.

Detailed job description and main responsibilities

• Working in a Health & Justice service in Worcestershire Outreach, with the flexibility of working across Worcestershire Health & Justice Services, dependent on skill and service need.  2.    Delivering a high quality short term intensive support service to patients and their families. 3.    Working towards developing a short term plan at the point of referral; this may be within Custody or Court settings, or within the community.  The plan will be based on needs identified during assessment by Health & Justice Practitioner in Custody or Court.  The aim of the intervention is to support and empower the individuals to reach identified goals and attend initial appointments with identified service.  This will involve the individual in planning and prioritising their needs.  4.    Will be responsible for liaising with multiple agencies, criminal justice system and other partners. 5.    Identify, assess and refer people of all ages with a wide range of mental health, learning disability, substance misuse and/or social vulnerabilities when they first come into contact with the youth and adult criminal justice systems on suspicion of having committed a crime. 6.    When a person is assessed as having single or multiple vulnerabilities, they will be referred to the appropriate treatment or support service and an appropriate package of care and/or support will be instigated.
