Job summary
Belgrave Medical Centre is seeking an enthusiastic, compassionate and proactive Health Care Assistant (HCA)/ Care Navigator to join our friendly and supportive team. This is an exciting opportunity for an experienced HCA who is passionate about improving patient outcomes and supporting the health and wellbeing of our local community.
The successful candidate will work closely with our GPs, nurses, pharmacists and wider multidisciplinary team to deliver high-quality patient care while supporting the smooth running of the practice. The role includes both clinical and administrative responsibilities, with a strong focus on patient-centred care, prevention and health promotion
Main duties of the job
Care Certificate
Accredited Phlebotomy Training
Previous experience working as a Health Care Assistant within General Practice or Primary Care
Excellent communication and interpersonal skills
Confident user of SystmOne
Strong organisational skills and ability to prioritise workload
Good understanding of confidentiality, safeguarding and patient-centred care
Ability to work effectively within a multidisciplinary team
Competent in accurate clinical record keeping and use of electronic patient records

Job responsibilities
Undertake phlebotomy, ECG, Care Plan, Mental Health clinics and other delegated clinical duties.
Carry out NHS Health Checks, blood pressure monitoring, height and weight measurements.
Support the completion of personalised care plans and health reviews.
Assist with patient care navigation and signposting to appropriate services.
Proactively support patients to improve their health and wellbeing within the community.
Maintain and manage stock levels, ensuring clinical rooms are fully stocked and organised.
Monitor vaccine and clinical stock supplies and support cold-chain processes where required.
Ensure accurate clinical coding and documentation within SystmOne.
Promote health education and preventative healthcare initiatives.
Support the nursing and clinical team with infection prevention and control procedures.
Deliver excellent customer service and provide a positive patient experience at all times

- Experience of working within Primary Care, Phlebotomist training completed, Care Certificate Completed, excellent S1 knowledge and managing your own HCA clinics.
