The General Practitioner (GP) working in the Primary Care Network (PCN) commissioned On the Day Service will be at the forefront of providing patient-centred, urgent care within the primary care setting.

This role is critical in ensuring patients presenting with acute medical concerns receive timely and high-quality consultations, advice, treatment, and referrals as needed. The GP will work collaboratively with a multidisciplinary team that includes nurses, healthcare assistants, pharmacists, and administrative staff to provide seamless and holistic care. Main duties of the job

Clinical Responsibilities

• Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation or Primary Care Network.
• Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
• Undertake detailed clinical assessments, including history-taking, physical examinations, and ordering diagnostic investigations when clinically appropriate.
• Screening patients for disease risk factors and early signs of illness.
• Formulate and implement evidence-based management plans tailored to each patient’s clinical needs, ensuring safe and effective prescribing practices.
• Manage medical emergencies within the scope of primary care, escalating or referring patients to secondary or specialist care services as necessary.
• In consultation with patients and in line with current disease management protocols, developing care plans for health.
• Providing counselling and health education.
• Referring to other care providers as appropriate.
• Recording clear and contemporaneous consultation notes to agreed standards.
• Collecting data for audit purposes and complying with QOF requirements.
