We are seeking to recruit a highly motivated and passionate Forensic Outreach Practitioner for the forensic services within Cheshire and Wirral Partnership NHS Foundation Trust (CWP).

A strong knowledge base in substance misuse linked with both mental health conditions and neurodevelopmental disorders is essential.

We are seeking a highly organised and hard working individual with a good knowledge of secure services and discharge planning.

The post holder will be required to have excellent communication skills and be able to work both autonomously and within a multi-disciplinary team.

Main duties of the job

The post holder will work primarily in The Alderley Unit (mental health low secure ward on the Soss Moss Site, Macclesfield) and Maple Step down ward (Bowmere hospital).

Their role within the discharge phase will be interchangeable based on the specifics of that case. The primary focus will be on overcoming challenges that prolong admission.

The post holder will help coordinate the assessment of patients for low secure beds held by CWP. They will work collaboratively with the MDT members from the referring team and the CWP Secure MDT to ensure this process is efficient and completed in a timely manner.

They will ensure that the person is kept at the centre of planning, taking into consideration their risk profile. The post holder will also work with patients and the MDT to plan for safe discharge from the secure service. They will liaise with agencies to support individuals and help coordinate the process.
