This is an exciting opportunity to join the Forensic Outreach Service in one of the dedicated clinical posts supporting a new forensic step-down supported living service, scheduled to open in January 2027.

The Service will be delivered as part of the SWLSTG Adult Secure discharge and step-down pathway, commissioned through the South London Partnership (SLP) Adult Secure Provider Collaborative Programme. The service will provide an integrated model of care and support through a partnership between the Forensic Outreach Service and a Voluntary and Community Sector (VCS) provider. The VCS partner will deliver the supported living pathway and work closely with the SWLSTG Forensic Outreach Service which will host dedicated clinical posts.

The Service is commissioned for adults stepping down from Adult Secure inpatient services into a community setting, whose ordinary residence and Section 117 aftercare provision sits with one of the five South West London and St George’s Mental Health NHS Trust (SWLSTG) boroughs, for health and social care. It will enable people to transition safely and sustainably from secure mental health services into the community, through access to high-quality accommodation and specialist, recovery-focused support delivered in a least restrictive setting.

Main duties of the job

This is a hybrid role within the Forensic Directorate, offering the opportunity to support individuals living in the forensic step-down service alongside a small caseload of patients receiving care in the community across South West London. The successful candidate will be an integral member of the Forensic Outreach Service, helping to promote recovery and community integration.

As a key member of our multidisciplinary team (MDT), you will manage a caseload of individuals with forensic histories, acting as their Care Coordinator and, where appropriate, Social Supervisor. Working alongside psychiatrists, psychologists, occupational therapists, recovery workers, and social workers, you will coordinate care that supports recovery, reduces risk, and promotes independence.

You will work collaboratively with a wide range of partner agencies, including the police, Ministry of Justice (MOJ), probation services, housing providers and residential care providers. A key part of the role will be working in partnership with the VCS provider to ensure care plans and risk management plans are updated promptly to reflect any changes in need.

Detailed job description and main responsibilities

To get to know all patients living in the forensic step-down service as well as the patients on the team caseload.

To review those on their personal caseload at the frequency agreed by the team.

To plan, implement and evaluate interventions delivered in the community, and participating in multidisciplinary care plans.

To ensure all care plans and risk assessments are updated promptly and shared with the Voluntary and Community Sector (VCS) provider.

To keep in contact with patients’ relatives, friends, carers and professionals in other agencies as appropriate.

To administrate, order and maintain a stock of medication and other medical sundries, in accordance with relevant policies and procedures.

To co-ordinate the care of those patients as the nominated care Coordinator

To act as social supervisor for those patients for whom the Care Coordinator is the nominated social supervisor, including producing pro-forma reports for the Ministry of Justice (after receiving appropriate training, if necessary).

To respond, sometimes at short notice, to requests to see the team’s patients either individually or jointly with other team members (e.g. when a patient is in the red zone).

To attend and participate in all zoning meetings unless absent by prior arrangement.

To attend and participate in weekly clinics held at the step-down service in collaboration with the VCS provider.

To attend CPA meetings or Tribunals where appropriate.

To occasionally participate in scheduled and emergency meetings held under the Multi-Agency Public Protection Arrangements.
