Job summary

The FDS Coordinator will actively manage the administrative processes attached to suspected prostate cancer patient pathways, ensuring a smooth and efficient service for both the multidisciplinary team and patients, in order to deliver a high-quality administration service, a positive patient experience, and excellent customer service. Provide an efficient and effective administrative service to a multidisciplinary team. Actively manage administrative processes and coordinate care associated with patient pathways, including validation, bookings and solving queries that arise. Ensure access and quality targets are met, identifying potential delays and escalating this to the operational management team. Provide patients with a primary point of contact between the multidisciplinary team and their colleagues, patients, relatives and GPs, providing relevant information and assisting with queries.

Main duties of the job

To collect and provide good clinical and care pathway data and information management, for use by MDTs and to meet Trust audit requirements. To work across the Trust and Networks - liaising with MDT teams and colleagues at all levels, to ensure the timely care of patients. To achieve Cancer Waiting Time targets To ensure timely delivery of care through tracking and navigating patients along the care pathway. Ensure access and quality targets are met, identifying potential delays and escalating this to a team leader or supervisor To achieve high quality service, through the Peer Review. Detailed job description and main responsibilities

  • To register suspected gynaecology cancer patients on Somerset, and track along the care pathway using Alex Health IT system.
  • Manage appointments for patients, in accordance with the Trust’s Elective Access policy. Where this is not possible (i.e. due to capacity issues or patient choice), escalate and take ownership for managing the issue with support from the operational management team.
  • Provide patients with a primary point of contact between the multidisciplinary team and their colleagues, patients, relatives and GPs, providing relevant information and assisting with queries.
  • Manage and answer telephone queries related to the service in a courteous and prompt manner.
  • Ensure any onco-alert queries are actioned in a timely manner by updating the escalation sheet and managing clinic utilization for Colposcopy/Hysteroscopy & Rapid Access..
  • Resolve complex queries, using analysis, experience and judgement to determine when to pass the caller on to a member of the clinical team, or when to escalate the call to a senior colleague. Where calls are passed on, ensure that the caller is reassured regarding expected response times and has further contact details if necessary.
  • Act as a point of contact for the department or specialty, dealing with queries from stakeholders and passing on relevant information to appropriate team members or departments as required.
  • Ensure administrative teams are responding to e-mail queries, monitoring, managing and triaging email correspondence through generic inboxes and message pool/patient portal.
  • Managing daily admin huddles and provide updates to operational management team.
  • Deal with stressful situations that may arise when dealing with patients who are anxious or distressed, or where there are barriers to understanding, using tact, diplomacy and empathy at all times and in line with Trust values.
  • Use developed communication skills in order to deal with patient concerns or complaints with empathy and understanding, finding a solution that assures the patient.
  • Identify potential delays or issues with a patient pathway or potential breaches, and take preventative steps to avoid these, i.e. chasing diagnostic reviews, escalating pending results of tests and capacity issues.
  • Proactively monitor the patient pathway in order to ensure that all elements of a patient’s care are arranged appropriately, and in a timely manner.
  • Identify areas for improvement and development in office procedures, develop and implement new systems to support service development.
  • Attend weekly cancer PTL along with the Assistant Service Delivery Manager/Service Delivery Manager & MDT Coordinator.
