Job summary

<p>As a <strong>Family Brief Intervention Worker</strong>, you will join our <span class="single_desc_int_gb_word">Child and Family Wellbeing Service in Warwickshire, working as part of the School Nursing Team to <span class="single_desc_int_gb_word">support</span> children, young people and families across the county</span>.</p><p>This is a rewarding community-based role where you will help deliver the Healthy <span class="single_desc_int_gb_word">Child Programme for children and young people aged 5 to 19 years, and up to 25 years for those with Special Educational Needs and Disabilities</span>. </p><p>You will work directly with children, young people and families, providing early intervention, health promotion and practical <span class="single_desc_int_gb_word">support that helps improve health outcomes and reduce inequalities. </span></p><p>Working across schools, community settings and family homes, you will play an important role in helping children and young people make positive lifestyle choices, improve their wellbeing and access the <span class="single_desc_int_gb_word">support they need to thrive.<a href="https://vco365-my.sharepoint.com/personal/sam_wright_hcrgcaregroup_com/_layouts/15/Doc.aspx?sourcedoc=%7B4BBCB86C-C3AC-4C80-903F-A3C34923604D%7D&file=Family%20Breif%20Intervention%20Worker.docx&
