We have an exciting opportunity for a Band 6 Nurse/Social Worker/OT to join us in delivering community mental health care to the Flexible Assertive Community Treatment (FACT) Team within East & South East Hertfordshire.
The area is well served with transport links to historic Hertfordshire and its glorious countryside as well as into London and beyond. We are looking for a Nurse/Social Worker/OT who is passionate about care delivering care to adults with mental health issues.
Main duties of the job
As a Social Worker in an Adult Community Mental Health Service (ACMHS) you will provide clinical expertise and leadership; participate in the referral and allocation process; safeguard vulnerable children and adults; contribute to case discussions and service related meetings; manage a caseload, provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control.

Detailed job description and main responsibilities
- As a member of the FACT Team, you will work as a member of the multi-disciplinary team, providing high care to adults of all ages experiencing mental ill-health. The core functions of the role will be to provide assessment, care planning and coordination of care to adults accessing the service. You will delivery recovery-focused care and support to service users and their carers that is focused on the principles of choice, control, and independence, as well as ensuring safety. · The post-holder will be responsible for the clinical and operational day to day service delivery ensuring that an individualised comprehensive assessment and Care Management service is effectively delivered. · To be accountable for delivering a quality service that is effective and efficient in line with the organisations objectives and performance indicators. · To contribute to strategic service development working collaboratively with other agencies Key Relationships. · You will hold a caseload which may comprise of adults experiencing a wide range of mental health and social care needs. You will ensure that you maintain constant reflection and development of your practice and will ensure your continuing professional development in line with the standards set out by the professional regulator.

Car driver essential as a majority of the posts are community based, the post holder will need a full valid driving licence and access to a car to use regularly for business purposes – including business insurance (unless you have a disability as defined by the Equality Act 2010).

ANALYTICAL SKILLS
Essential
- Ability to analyse service user and carer information
- Experience of undertaking small scale audits and surveys
COMMUNICATION SKILLS
Essential
- Excellent ability to communication with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing
- Ability to manage highly sensitive information
- Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports
AREAS OF EXPERIENCE AND KNOWLEDGE
Essential
- Post qualification experience
- Knowledge of legislation, theory and policy in relation to relevant care group
- Experience of undertaking assessments, focussed as personalisation, choice and control, and care planning
- Understanding and experience of personal budgets and direct payments
- Experience of multidisciplinary working and ability to work collaboratively
- Experience of partnership working with service users and carers, demonstrating dignity and respect
Desirable
- Experience of work as Approved Mental Health Professional and/or Best Interests Assessor
KNOWLEDGE, TRAINING AND EXPERIENCE
Essential
- An appropriate professional social work qualification
- Education to degree level
- Registration with Social Work England
- Training in principles and application of outcomes focussed assessments and personalization
- Relevant post-qualifying training for social workers
- Training and experience in Safeguarding
Desirable
- Approved Mental Health Professional/Best Interests Assessment Training
