An exciting opportunity has arisen for a Band 5 Enhancing Lives Navigator, within The Harm Reduction Team of Substance Misuse Services in the Central  region (Conwy / Denbighshire).

The post holder will work as an autonomous practitioner carrying a caseload of people who are experiencing complex needs in relation to issues including substance use, homelessness, mental health and offending behaviour, which is often underpinned by trauma.

The aim being to ensure the best possible outcomes for those people experiencing complex needs through effective and sustained engagement and the use of a multi-disciplinary and person centred approach.

Navigator activities take place using an assertive outreach approach in a wide range of settings including, on the street, hospital, clinic, community settings and in people’s accommodation (emergency/temporary).

Main duties of the job

  • Provide high quality support, care and advice to service users that focuses on their strengths and goals and is delivered in a way that suits them and within a framework of active engagement, positive risk taking and person-centred planning.
  • To manage own case load through planning, prioritising and carrying out required support activities, being adaptable as individuals circumstances and presentation change; demonstrating effective caseload and time management skills. To use own judgement and analyse assessment information from a range of sources and situations in order to develop support to meet individual’s
  • To carry out comprehensive assessments  which support identifying the needs of the individual.
  • Support individuals to identify their strengths and  goals through assessment of physical, psychological, cognitive, emotional, environmental and social factors; devising, writing and evaluating action plans.
  • To actively engage individuals experiencing complex needs in the Enhancing Lives process, using a range of communication styles and approaches which support them to take responsibility for decision making and goal setting. To identify and challenge barriers that individuals experiencing complex needs face in accessing services and support; acting as an advocate for the individuals being supported.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
