This is an exciting and autonomous role supporting the urgent care response plan in Cornwall working within the Hospital at Home Service This is a role requiring an innovative and experienced clinician, with a proven track record or recognised potential for embracing and leading change. This role will require vision and enthusiasm to help codesign and transform our evolving community services within Cornwall as we look forward to embracing Integrated Neighbourhood Teams. There is a need to test new models of care delivery and enhanced practice roles, this will necessitate a flexible and responsive individual who is confident to adapt and work with autonomy and ambiguity.

Main duties of the job

You will be required to consult, assess, and treat patients who are referred with undifferentiated, unpredictable, varied and complex needs, ensuring holistic assessment, prompt treatment and crisis care planning is undertaken to directly prevent unnecessary acute hospital admission.

The enhanced clinical level of this role will require you to be able to undertake enhanced level comprehensive assessment including history taking, physical examination of the patient and a robust systems review. You should be able to assess and manage frailty and complex chronic disease, identify deterioration and implement immediate strategies to reduce the risk of acute admission. There is also a requirement to support patients who are palliative or coming to the end of their life. Furthermore you will need to be able to recognise the early symptoms of disease exacerbation, acute illness and injuries, based on an understanding of chronic diseases, the disease process, current evidence and practice standards.

Essential

  • Ability to work autonomously and flexibly, managing competing priorities

Knowledge and Abilities

Essential

  • Knowledge and understanding of issues relating to frailty and long term conditions
  • Understanding of Palliative care and deterioration clinical support required for patients in the last year of life.
  • Ability to implement research into practice

Education / Qualifications and Relevant Experience

Essential

  • Substantial experience at a senior level working with older people, assessing and managing individuals with long term conditions and frailty and people in the last year of life.
  • Completion of/or currently undertaking the Degree level long term conditions module, or RCN accredited Nurse Practitioner diploma or the equivalent. Or evidence of undertaking advanced clinical examinations and making independent clinical decisions for complex and unpredictable presentations.
  • Current professional registration as an RN or AHP with NMC or HCPC

Desirable

  • Current Non-medical Prescriber or currently /willing to undertake the module
  • Experience of leadership and leading change or project implementation
