The post holder will work as an integrated member of the multi-disciplinary team (MDT), working to support patients who present with complex and challenging enhanced care needs. The post holder will be required to use their initiative when working on the wards to implement person centered plans of care and are expected to report any clinical progress or change to the Nurse in Charge. The post holder will carry out assigned tasks involving direct/indirect service user care and support service users under the supervision of a RN.

Have the ability to utilise de-escalation techniques in challenging scenarios as required
May require the use of restraint in extreme circumstances
Detailed job description and main responsibilities
- Provide reassurance and support to patients and carers/family • Enable patients to participate in recreation and leisure activities • Support/enable patients to maintain their personal hygiene and appearance • Support/enable patients to eat, drink and maintain personal care • Support/enable patients to achieve physical comfort • Support/enable patients to manage their domestic and personal resources • Make written records under the supervision of an RN • Recognise and respond appropriately to urgent and emergency situations • Provide escort for therapeutic leave for service users • May be requested to move to another ward within the Trust • Give verbal reports on patients care, needs and daily outcomes • Contribute to the support of patients during programmes of care/rehabilitation • Provide 1:1 support for patients to maintain safety Communication and Relationship Skills • Support individuals equality, diversity and rights • Ensure a friendly and helpful attitude to patients, visitors and colleagues, at all times. • Enable patients to maintain contacts in the community • Maintain effective working relationships with the multidisciplinary team. Interviewer information pack • Provide verbal feedback to colleagues regarding patient presentation during for example, handover. • To attend and participate in handovers • Interacting with patients which may involve providing information and using skills to overcome barriers in an empathetic and reassuring manner Analytical and Judgmental Skills • Monitor patient health and well-being and report any changes to the Nurse in Charge/Team Leader Planning and Organisational Skills • Work as part of a multi-disciplinary team in planning • Undertake Safety and Security (SAS) duties if trained to do so. Physical Skills • Can undertake de-escalation techniques Responsibilities for Policy and Service Development • To work within Trust and local policies and procedures Responsibilities for Finance • Handle patient’s cash and property in line with the Trust’s Standing Financial Instructions Responsibilities for HR Interviewer information pack • May support student nurses and learners on the ward • May participate in the internal induction and demonstration of own duties to new staff Responsibility for Information Resources • With support/guidance from the registered nurse can update patient records Research and Development • Participate in internal audits Freedom to Act • Works semi-autonomously during escort duties, ability to work with a minimum of direct supervision.
