We are looking for a compassionate and motivated nurse to join our team as an End-of-Life Care Clinical Nurse Specialist, embedded within NEXUS Primary Care Network (PCN), Exeter.
Main duties of the job
This is an exciting opportunity to be part of an innovative service that helps people receive the right support earlier, giving them more time to plan, make informed choices and receive care that reflects what matters most to them.
You will build trusted relationships with people, families and professionals, supporting meaningful conversations about future care, coordinating support and helping ensure wishes and preferences are understood and respected.
A key focus of the role will be supporting the timely completion of Treatment Escalation Plans (TEPs), ensuring individuals are empowered to make informed decisions about their care, treatment and future wishes. You will champion advance care planning and work collaboratively with patients, families and professionals to ensure care is aligned with what matters most to each person.

Job responsibilities
Working alongside GP practices, community services and Marie Curie colleagues, you will help improve experiences of care, reduce avoidable crises and support people to remain in their preferred place of care wherever possible.
Most importantly, you do not need to see yourself as an end-of-life care specialist already. If you are an experienced nurse who is passionate about person-centred care, curious to learn and committed to making a difference, we will support you to develop the knowledge, confidence and skills needed to thrive in the role.
Hours: 37.5 hours per week, Monday to Friday
Contract: Permanent (flexible working to be discussed)
Salary: Band 6, £39,959 - £48,117 per annum (aligned to NHS Agenda for Change)
Location: Exeter Primary Care Network and community settings
