Are you an enthusiastic and highly motivated District Nurse with a Specialist Practice Qualification, looking for an opportunity to lead and develop our friendly and supportive Lichfield & Burntwood District Nursing team?

We are looking for a NMC Registered Band 6 District Nurse with a Specialist Practitioner Qualification in District Nursing who is dynamic, motivated and highly skilled to support, coach and develop individuals to deliver consistently high-quality services.

You will be managing a caseload of patients while providing support and supervision for team members and students to enable provision of high-quality person-centred care to patients within their own homes which includes ensuring high standards of care are delivered in line with MPFT’s policies and meeting locally required targets.

You will be well supported by our Lead Specialist Practitioner and Community Nurse Educator.

This role is part-time, 30 hours per week across a 7 days per week service with an expectation to travel independently across the geographical patch.  We will consider requests for flexible working.

Main duties of the job

As Band 6 district nurse you will have a detailed professional and clinical skills and knowledge, be a skilled and competent specialist practitioner and have a focus in the following areas:

  • Palliative care
  • Administration of medication
  • Management of long-term conditions
  • Onward referrals to other members of the Multi Disciplinary Team including Specialist Pelvic Health, Community Matron, Diabetes Specialist Nurse and therapy services
  • Communication with GP’s, Palliative Care teams
  • Assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support
  • Supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.

Work with the Operational Lead undertaking delegated tasks in team management
