The Ealing Hospital Discharge Hub has an exciting opportunity for an enthusiastic and motivated individual with care experience and experience within hospital discharge pathways, discharge planning and working within a multiple disciplinary team. Your main role will be to support P1 discharges from Ealing Hospital into the community in a safe and timely manner, hold an active daily caseload of patients and to ensure they are discharged within the agreed time frame and complete pre checklists for P1 patients being discharged. The successful post holder will work in partnership with colleagues in the acute sector, social services, Ealing Community Partners, ICB and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.
Main duties of the job
To support P1 discharges from Ealing Hospital into the community in a safe and timely manner, hold an active daily caseload of patients and to ensure they are discharged within the agreed time frame and complete pre-checklists for P1 patients being discharged. • To work as a core member of the acute based Ealing Hospital Discharge Hub team to build and deliver the shared vision for a hospital discharge service. • Be responsible to optimize hospital bed utilization and reduce the length of stay by ensuring the timely, safe transfer/discharge of pathway 1 patients to the most appropriate and agreed environment. • To act as a pro-active community health service advocate, on behalf of West London NHS Trust, through extensive knowledge of relevant community-based pathways within the area of operation and ability to support effective discharge planning. • Work in close partnership with hospital colleagues and external agencies such as Social Services, ICB, Housing Associations, GPs and voluntary organisations. This will involve attending meetings outside the Trust and sharing Care Management Plans with the ASC and other community providers.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

Knowledge
Essential
- Understanding of Hospital discharge and community support guidance
- Working knowledge of NHS 10 year plan
- Experience of working with D2A pathways
- Knowledge of frailty care provision
Desirable
- Broad medical knowledge including end of life, chronic conditions and frailty
- Understanding of the care act assessment
Experience
Essential
- Experience of supporting discharges for individual patients in the acute or community bedded units
- Experience of implementing care plans
- Working within a Nursing, Physiotherapy and Occupational therapy team within an inpatient or community setting
- Experience of providing face to face care and therapy support
- Experience of participating in governance cycle

- Experience of working within services that are going through transitional periods
- Working within acute discharge teams
Personal Qualities

- Sound IT skills and experience with Cerner/Optica/Systm1
