Sherwood Forest Hospitals are looking for dedicated and proactive Discharge Coordinators to join our dynamic team. This is your opportunity to play a vital role in patient care and hospital flow.
What You’ll Do:
- Support the Discharge to Assess process, ensuring safe and timely discharges.
- Help maintain flow and capacity across the Trust.
- Work across all clinical areas and provide cover at King’s Mill Hospital, Mansfield Community Hospital, and Newark Hospital.
What We’re Looking For:
- Excellent communication skills and a calm, adaptable approach.
- Self-motivated and able to respond quickly to changing situations.
- A clear understanding of discharge pathways and awareness of social care and health processes (full training provided).
- A full driving licence is essential as you’ll travel between sites.

Working Pattern:
Seven-day service, including bank holidays.
Shifts include 8:00 am–4:00 pm and 8:30 am–4:30 pm.
