Main duties of the job
In this autonomous role, you will provide a high-quality, evidence-based service for people living with diabetes, working within Trust policies, national guidelines, and your professional scope of practice. You will play a key role in improving patient outcomes by promoting best practice in diabetes care, reducing the risk of diabetes-related complications, and empowering individuals to take an active role in managing their condition.
You will be responsible for assessing, planning, implementing, and evaluating personalised care and treatment plans, ensuring patients receive timely access to appropriate care pathways and specialist referrals.
As a clinical expert, you will provide specialist advice and support to patients, carers, and healthcare professionals, while delivering education and training across multidisciplinary teams to enhance diabetes knowledge and skills.
You will also have the opportunity to initiate and adjust injectable and oral diabetes medications, supporting patients to manage their health and wellbeing more effectively.
The successful candidate will contribute to the ongoing development of the service through participation in clinical audit, quality improvement initiatives, and service evaluation activities.

Essential
- Able to set priorities and organise own time
- Demonstrable use of evidence-based nursing to develop practice
- Demonstration of excellent interpersonal skills
- Ability to travel to different geographical locations across the Trust footprint in line with service needs
Desirable
- Full UK driving license with willingness to use own car for work purposes
Knowledge, Training & Experience
Essential
- Evidence of diabetes care experience
- Experience of training or mentoring nurses and other health professionals
- Experience delivering patient / staff education
- Knowledge of the management of diabetes mellitus
Desirable
- Experience of working in the community
- Experience of liaising between primary and secondary care
- Experience of working within a multi-disciplinary team
- Able to manage own consultations
- Participates in an active teaching role both formal and practical
Qualifications & Professional Registration
Essential
- Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2)
- Degree / Diploma in Nursing studies or equivalent
Desirable
- Non-Medical Prescribing
- Diabetes qualification / evidence of study
