This role is for our Inpatient Diabetes Services at Frimley Health NHS Foundation Trust
As part of the exciting and innovative developments across Frimley Health NHS Foundation Trust, we are delighted to offer a Band 6 vacancy for an enthusiastic and committed Diabetes Nurse Specialist to join our expanding team.
In this role, you will be instrumental in supporting the delivery of our 7-day inpatient diabetes service, contributing to the day-to-day clinical care, education, and support of patients with diabetes across our hospital sites. You will work alongside a friendly, multidisciplinary team including consultants, dietitians, and other specialist nurses.
About You: We are looking for a qualified, NMC-registered nurse with a strong interest or experience in diabetes care. You should be confident in working autonomously within a team, have excellent communication skills, and a desire to develop your clinical expertise in a specialist setting.

Main duties of the job
Key Responsibilities:
Provide daily reviews of inpatients with diabetes, identifying and addressing clinical needs in a timely and effective manner.
Participate in inpatient follow-up clinics, supporting patients in managing their condition during hospital stays and after discharge.
Develop and implement patient-centred care plans, offering guidance on blood glucose monitoring, medication regimes (including insulin), and lifestyle interventions.
Manage the telephone advice line and email service, offering responsive support to both patients and staff seeking diabetes-related advice.
Ensure that electronic and written documentation is accurately maintained, reflecting care delivery and supporting safe, coordinated practice.
Participate in ward-based education and training, promoting best practices in diabetes management among ward staff.
