An exciting opportunity has arisen for a highly motivated, enthusiastic, and reliable Band 5 Diabetes Patient Education Co-ordinator for maternity cover
You will be managing a small team of nutritionists and administrators, to support with the co-ordination of diabetes education whilst also delivering some groups and one to one support.
You will use leadership, organisational and evaluation skills to support effective service delivery throughout Gloucestershire, which enhances patient access, uptake, and outcomes.
We work closely with colleagues across the healthcare system and the voluntary sector, to provide well-rounded support to our patients.

Main duties of the job
- To co-ordinate the delivery, development and provision of the Diabetes Structured patient education programme for Gloucestershire.
- To deliver group education to people with Type 2 Diabetes and their families/carers.
- Support the person with diabetes to develop self-management skills with the support of registered nurses, dietitians and nutritionists.
- Offer encouragement and support to enable the person with diabetes to identify suitable activities that support a more active lifestyle, according to individual needs and abilities.
- Manage group dynamics and support collaborative working with internal and external agencies.
- Carry out self-evaluation after delivery of every education course to ensure key objectives are met.
- Carry out service evaluation and audit to improve the uptake and access of patient education with the goal of improving health outcomes for the population. The qualification, training & experience requirements for the role are underlined in the Job Description/Person Specification.
Detailed job description and main responsibilities
Co-ordinating the delivery of Diabetes Structured Education in Gloucestershire.
Delivering Diabetes and You and Diabetes Food and You education sessions.
Calls to service users to arrange appointments, follow up sessions and signposting.
Representing the Community Diabetes service for Patient Education related meetings/work and reporting to the ICB.
Participating in local audit and evaluation of the programmes
Participating in QISMET accreditation in line with accreditation cycle; this will include collating and submitting documentation and liaising with the accreditation team.
Development and peer supervision of programme delivery.
1-1 consultations for those unable to engage in a group setting.
Promotion of the sessions in the local area, including using social media platforms. Line management of patient education team, to include annual appraisals and 1-1 reviews.
Co-ordination of Annual leave and study leave.
Identifying training needs within the team.
Responsible for maintaining patient resource stock levels.
Responsible for risk assessment of venues and ensuring adherence to local guidelines.
