Dementia Support Worker

Job summary

An exciting opportunity to join The Care Home Education and Support Service in Torbay as a Dementia Support Worker has arisen following service development.

We provide specialist guidance, education and intervention to local residential and nursing care homes, helping staff deliver safe, confident and compassionate care for people with dementia and who present with the associated behavioural and psychological symptoms of the condition.

This role is an opportunity to make a meaningful difference to residents wellbeing while empowering care home teams with the skills and knowledge they need.

As part of CHESS, you will work closely with care home staff, residents and families to promote best practice, improve outcomes and strengthen the quality of care across Torbay. Youll have experience of working with this highly specialist client group and in the delivery of supportive, multidisciplinary team and personcentred care.

Main duties of the job

• To complete holistic, comprehensive assessments of patients with an established diagnosis of dementia and the associated biological, psychological and environmental care needs.
• To consider interventions for people with advanced dementia, frailty who may present with multiple co-morbidities and challenges.
• Make recommendations to care homes and the wider health and social care network in relation to therapeutic strategies to optimise mental health and wellbeing.
• To communicate effectively with people with dementia and those caring for them and the challenges this may present.
• To model positive interventions and planned activity programmes to improve the quality of life of those under our care and to educate care home staff.
• To offer emotional support, advice, information and education where appropriate to people using services, carers and their family including signposting them to other services.
• To foster and maintain good working relations with members of other community teams, health and social care workers and inpatient services. Receiving and sharing information from GPs, carers and other agencies.
• To provide information and assistance to people using services and their carers so that they are aware of their rights, entitlements and the contents of their recovery care plan.
• To communicate any changes in the persons presentation to the team in a timely manner, so that early intervention can be provided.

To communicate in a skilled and respectful manner with clients, families and carers who may have specific difficulties in understanding and communicating and who may be distressed or present with challenging behaviour.

To participate in clinical meetings and to work collaboratively with other colleagues in the multi-disciplinary team.

To maintain appropriate professional boundaries whilst recognising abilities and limitations of own competence

To communicate complex and sensitive condition related information concerning the assessment, treatment and risk factors

To establish and maintain clear and professional relationships with others at all times.

To liaise and work collaboratively with a range of other professionals in statutory and non-statutory organisations agencies.

Provide a high standard of written and verbal communication that is clear and in line with Trust documentation standards.
