Job overview

The CYP Continuing Care Assessor is an experienced and confident clinician responsible for leading complex assessments for children and young people with significant, ongoing health needs. The postholder plays a key role in determining eligibility for Children & Young People’s Continuing Care, ensuring that every assessment, rationale, and recommendation is clinically robust, defensible, and firmly aligned with the national CYP Continuing Care Framework. This role sits at the heart of providing safe, person-centred, and equitable support for families. The Assessor undertakes detailed holistic assessments, brings together multidisciplinary evidence, and produces high-quality reports that stand up to scrutiny from partners, families, and governance processes. They manage cases that are often sensitive, fast-moving, and multi-agency in nature, requiring sound judgment, strong communication skills, and a calm, balanced approach. The postholder works closely with families, local authority colleagues, education teams, safeguarding, and a wide range of health professionals to build a joined-up understanding of needs and ensure care planning is clear, coordinated, and proportionate. They also contribute to quality assurance, help develop practice standards, support junior staff, and promote consistency across the pathway.

Main duties of the job

The Manager provides operational management, coordination, and leadership within a defined area of responsibility. Working closely with the Senior Manager and Head of Service, the postholder ensures that services and projects are delivered effectively, efficiently, and in line with agreed priorities. This will involve the development and presentation of key data, information and updates to groups of stakeholder and staff. The role involves managing teams, coordinating workloads, monitoring performance, and ensuring that quality standards are met, adjusting plans as required. The postholder supports improvement initiatives, encourages innovation, and promotes a culture of continuous learning and collaboration. Through effective communication, planning, and problem-solving, the Manager ensures that the ICB’s services contribute to high-quality, equitable, and sustainable care for the population. Ultimately, the Manager provides reliable operational delivery, visible leadership, and professional support to colleagues — helping the organisation deliver on its mission to improve outcomes and reduce inequalities for local people.
