Job summary

Geriatric Medicine Consultants are required for the expansion of OPAL, FSDEC & FAU services at Good Hope Hospital, part of University Hospitals NHS Foundation Trust.

We are looking for highly motivated and skilled individuals to join a genuinely innovative frailty service at an early stage of development, giving you an opportunity to shape FSDEC, OPAL and FAU pathways from the outset.

This post sits at the centre of a whole system frailty pathway and aims to deliver early Comprehensive Geriatric Assessment (CGA), reduce avoidable admissions, improve ED flow, shorten length of stay, and support safe discharge aligned to national acute frailty standards. Including:

*Delivering consultant-led frailty assessment and decision making for the Frailty SDEC operational from 08:00--20:00, 7 days per week, via a multi consultant rota (3.55 WTE).

*Working closely with the Frailty Navigator and OPAL team to provide early identification, triage and streaming at the hospital front door.

*Providing senior input to the Frailty Assessment Unit (FAU) for short stay (24--48 hour) geriatric assessment and stabilisation.

*Participating in the daily 11:00 North Locality Frailty MDT, involving community GPs, frailty nurses, and acute medical team to ensure safe and coordinated transitions of care.

Main duties of the job

Total: 10 PAs (WTE)

Final job plan will be agreed with the post holder upon appointment.

Direct Clinical Care -- 8.0 PAs

*Frailty SDEC sessions (working within 08:00--20:00 span via consultant rota)

*OPAL front door frailty assessment and streaming

*Daily 11am North Locality frailty MDT

*Time allocated for multidisciplinary collaboration and communication

*Supervision of resident doctors and allied health professionals

Supporting Professional Activities -- 2.0 PAs

*CPD, appraisal, revalidation

*Teaching and educational supervision

*Clinical governance & audit

*Research and innovation

*Service development

Optional Additional PAs

*Acute medicine on call (if mutually agreed)

*Additional service development or clinical leadership responsibilities

On Call (Optional)

*This post is not primarily designed for acute GIM on call, but on call can be incorporated where appropriate, depending on applicant preference and service requirements.

*Additional PAs will be discussed during job planning.
