Job overview

We are looking for an enthusiastic well-motivated Consultant colleague to join the dedicated team of physicians at the Princess Royal Hospital (PRH) in Haywards Heath in this new post.

The appointee will contribute to general and specialist clinics in diabetes and endocrinology, with some flexibility depending on their interests, skills and experience. They will participate in the care of general medical and specialist in-patients, and out-of-hours cover for the general medical take. General medical ward cover will be shared between four Consultants, with responsibility for patients on the diabetes / endocrinology / general medical ward and allocated medical outliers, and review of other specialty admissions as required. The appointee will share a 1:9 responsibility for general medical admissions on weekends, weekdays and Bank holidays.

Applicants should be enthusiastic teachers and trainers, keen to participate in our teaching programme in endocrinology and diabetes for students at Brighton and Sussex Medical School, and to the supervision and training of junior doctors. Research interests are supported and encouraged. Appointees will be encouraged and supported to take on a range of roles within the department, including clinical governance, quality improvement and audit, and service development.

Main duties of the job

The post holder will contribute to the delivery of high-quality Diabetes and Endocrinology services across both inpatient and outpatient settings. Clinical duties will include:

• Delivery of Diabetes and Endocrinology outpatient clinics and participation in relevant MDTs (see indicative timetable below)
• Shared responsibility for inpatient ward care and specialist advice
• Participation in weekend, bank holiday and evening working arrangements as outlined within the rota
• Shared responsibility for Advice and Guidance requests and triage of referrals
• Supporting the wider multidisciplinary team across the service
• Clinical supervision and support for junior doctors attached to the team
• Participation in audit, clinical governance, quality improvement and service development activities

On-Call Commitment

• 1:8 Category B on-call rota with a 2% supplement
• Subject to workforce review, this may reduce to a Category B rota with a 1% supplement

Detailed job description and main responsibilities

The job plan will be based on 10 PAs to include 8 DCC PAs and 2 SPAs, including participation in a GIM rota, detailed below.

Direct Clinical Care (DCC) – 8 PAs

There will be a varying frequency of out-patient clinics per week depending on whether the individual has ward commitments or not, as outlined in the indicative timetables. DCC includes general and specialist diabetes and endocrinology clinics, and participation in MDT or clinical supervision meetings (face-to-face or via video-link).  Clinic templates generally allow 15-20 minutes for follow-ups and 30 minutes for new patients, with a 2:1 – 3:1 ratio.

All Consultants provide clinical advice for members of the diabetes and endocrinology multi-disciplinary team. Other shared clinical duties include triage of referrals and our specialty ‘Advice and Guidance’ service for GPs.

All Consultants contribute to general endocrinology and diabetes clinics. There will be some flexibility about the allocation of sub-specialist work, depending on the interests and expertise of the appointee, but we would welcome contributions to any of the following sub-specialist areas of diabetes and endocrinology:

• Multidisciplinary diabetic foot clinics, working with our specialist diabetes podiatrist, and colleagues in orthopaedics and vascular surgery.
• Community diabetes supervision.
• Antenatal diabetes clinics.
• Diabetes pump clinics.
• Pituitary MDT and clinics.

The DCC includes a 1:9 general medical on-call rota for evenings and weekends. The acute medical take is run by the acute medical team (9-5 pm) and therefore a day on call involves the review of any patients not seen by the acute physician from 5-7 pm. The rest of the on call is from home overnight followed by a post take ward round at 8 am the following morning. A weekend comprises mid-take ward rounds on Friday, Saturday and Sunday, with post-take ward rounds on Saturday, Sunday and Monday, and on-call from home between ward rounds. Time off in lieu will be scheduled after a weekend on-call. The model of weekend working may change going forwards with general medical Consultants participating in an ‘acute medical model’ of care on weekends: the weekend on-call frequency would go down to reflect the increased workload when on duty.

The DCC also includes shared responsibility (between 4 Consultants) for a general medical / diabetes in-patient ward, medical outliers and medically fit complex discharge patients on other allocated wards (approximately 20 patients in total). Consultants on ward duties are expected to provide daily support to inpatients with reviews of new admissions to the ward on a daily basis as well as holding an MDT board round of all patients under their care. They are also expected to provide specialist consultations for in-patients in relation to diabetes / endocrinology, as required.

On Call Commitment: 1:8, category B, 2% (subject to workforce review may reduce to category B, 1%)

There is an SpR, an IMT3, and a ward team of FY1/FY2/IMT/Clinical Fellow (CF)/GP trainees attached to the firm, working an annualised rota.
