Job summary

Our Cardiology team is made up of a wide range of enthusiastic and passionate individuals including 15 consultants, 2 Specialist Doctors, 4 Specialty Doctors, 4 Specialty Registrars, 3 internal medicine trainees and 3 Foundation trainees. We have invaluable support from a team of specialist nurses based both in our trust and in the community. We invite applications for consultants in cardiology with a specialist interest and formal training in Heart Failure. Our vision is to deliver patient centred, accessible and integrated services, as seamlessly as possible with primary care. With 11 Coronary Care and 26 Cardiology beds, new diagnostic facilities, and access to a full range of non-invasive and invasive cardiac facilities our team has access to exceptional, modern facilities as well as general imaging, including TTE and TOE, CTCA, CMRI (including stress MR) and Interventional radiology services. We have a fully established bradycardia pacing, complex device and PCI service (excluding PPCI provided in the Tertiary centre). We have an established Nurse-led heart failure service with community integration. We are excited to include in this post some GIM training time for those clinicians who are dual accredited in Cardiology and GIM. Within the job plan for this position are up to 1.5 PA of DCC to General Internal Medicine (GIM), which is to enable the successful applicant to be part of the team delivering inpatient care to general medical inpatients.

Main duties of the job

This post is offered on a full time (10 PA) basis although less than full time applications will be considered. Duties will be based primarily on our Wakefield (Pinderfields) site although all our medical staff are involved in delivering care to patients on all three sites across the Trust. The main duties of the post will include: Providing, in conjunction with Consultant colleagues, an acute cardiology service to the Mid Yorkshire Teaching NHS Trust. Inpatient cardiology work including the 11 bedded CCU and 28 bedded cardiology ward Urgent and Emergency Care duties for cardiology at Pinderfields out of hours is currently assessed as 0.45PA per week, with local weekday on-calls (1:15), and weekend on-calls (1:12). This equates to an 3% availability supplement. Time off in lieu will be arranged via the job planning process The Department runs 2 cardiologist of the week rotas on a 1 in 13 basis; - One for the 11 bedded CCU (which includes accepting referrals from the Acute Admissions Unit and from inpatients elsewhere in the hospital) - One for the 28 bedded cardiology ward.

Within the job plan for this position are up to 1.5 PA of DCC to General Internal Medicine (GIM), which is to enable the successful applicant to be part of the team delivering inpatient care to general medical inpatients.

The new department of GIM, within the Specialty and Integrated Medicine Clinical Service Unit, will be staffed by 15 dual-accredited physicians from Group 1 specialties. These consultants will work a consultant of the week model of care on a one-in-eight rota, providing a daily consultant ward round (4-hour duration). The ward will have 2 consultants each week doing a daily ward round, the consultants will change every week. Ward is based on one site.

Time off in lieu will be arranged via the job planning process • The Department runs 2 “cardiologist of the week” rotas on a 1 in 13 basis; - One for the 11 bedded CCU (which includes accepting referrals from the Acute Admissions Unit and from inpatients elsewhere in the hospital) - One for the 28 bedded cardiology ward. • The current Heart Failure Service consists of 2 consultants, a Heart Failure Team Manager (0.4 WTE), 2 WTE band 7 Heart Failure Nurses, a 0.9 WTE band 7 Heart Failure Nurse, 1 WTE band 6 Heart Failure Nurse and a 0.8 WTE band 6 Heart Failure Nurse. The service provides in-reach to the Pinderfields Hospital Site, care of Heart Failure patients in the community, in addition to clinics in heart failure management and complex-devices. The successful candidate will be expected to support ward rounds and provide specialist opinions for outlying patients in the wider trust. The successful candidate will be expected to deliver outpatient clinics. • Participate in, and help further develop the heart failure service in conjunction with the team of 2 other Heart Failure consultants. • Participate in inpatient care of cardiology, and GIM patients if appropriate. • Participate in outpatients (general and specialist) including providing advice to primary care colleagues via “e-consultation” on our shared care referral pathway. • Supervision and teaching of the junior staff (SPA time allocated according to the roles commissioned)

Essential person specification requirements

To meet the requirements of this role, applicants must demonstrate the following essential criteria:

Full GMC Registration with licence to practise

Inclusion on the GMC Specialist Register for Cardiology or within 6 months of obtaining CCT (or CESR / equivalent) with sub-specialty training in coronary intervention.

Medical Degree from recognised medical school MRCP or equivalent CCT (or CESR / equivalent) with sub-specialty training in heart failure

Sufficient experience at Senior level to be placed on the Specialist Register Evidence of formal training in advanced heart failure Wide experience in general Cardiology and acute assessment environment, in particular the management of patients with heart failure Computer literate Wide experience of NHS procedures and practice Eligibility to work in the UK

Demonstrates intelligent and analytical approach to problem solving. Good time management

Possesses good communication skills. Flexible & adaptable

Ability to work closely with Consultant colleagues Caring attitude Good ‘team player’ Willingness to provide and learn all aspects of hospitals internal systems and processes and adhere to local policies Physically able to carry out the duties as specified in the job description and ability to evidence competencies Disclosure + Barring Service (DBS) Clearance Driving licence
