We are offering an exciting opportunity for a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) Team based in Tamworth.  The Team is an integrated clinical and care and support service providing short term reablement intervention to enable people to maximise their independence and meet their health and social care needs.

You will be responsible for scheduling and allocating care calls on the Complex Discharge Tracker System and will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales and will ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.

We are looking for a team member who is enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills.  There will be a requirement to deliver face to face care/support to our community patients on an adhoc basis. The post holder is expected to commute to the office bases across South Staffordshire, if required. Shift patterns are on a rotational basis over 7 days, early shift 7am – 3pm and late shift 12pm – 8pm working, therefore flexibility is required for this role.

Main duties of the job

As a Complex Discharge Tracker, you will review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.

As part of this role, you will be expected to:

  • Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
  • Attend all CDT training and briefing sessions as the system evolves.
  • Ensure that all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.
  • Provide care/support to community patients in their own homes on a adhoc basis.
